Why You Got the Form in the First Place

Not every Medi-Cal beneficiary receives an MC 216. The MC 216 arrives when that automatic check fails. Usually that means your income can’t be verified through the databases, records are incomplete, or something in your household changed enough that a caseworker needs you to confirm the details in writing. What the MC 216 Asks You to Confirm

  • Contact information, preferred language, and any authorized representative acting for you.
  • Household members, including your spouse or registered domestic partner, children, parents in the household, and anyone claimed on your federal tax return. This is also where you report a marriage, birth, move, incarceration, or death from the past year.
  • Tax filing status: whether you plan to file a federal return, your filing status, and whether anyone claims you as a dependent.
  • Income for each household member, including the amount before taxes, how often it’s received, when it started, and whether it will continue. Variable income is reported as your expected total over the next twelve months.
  • Expenses and deductions such as self-employment costs, student loan interest, IRA contributions, and alimony.
  • Medicare number and monthly premiums for Parts A, B, C, and D, if any.

  • Other health insurance you carry, with the insurer’s name, coverage type, and premium.

Filling It Out Without Creating Problems

Sign and date the declaration. An unsigned form is treated as incomplete and will either delay processing or generate a follow-up request.

The Asset Section (Only Some Enrollees)

If you’re enrolled in a non-MAGI Medi-Cal program, meaning you qualify because you are 65 or older, blind, disabled, or receiving long-term care, your renewal will include property questions. Beginning January 1, 2026, California reinstated asset limits for these programs: $130,000 for one person, $195,000 when both members of a couple apply, and an additional $65,000 for each extra household member. When one spouse is in a nursing facility and the other lives at home, the community spouse can hold up to $162,660 under the Community Spouse Resource Allowance.

Everyone else, meaning most working-age adults and children in MAGI-based Medi-Cal, does not have an asset test and can skip this concern entirely.

For those who do report assets, several categories are excluded from the count: your principal residence, household goods and personal effects, one vehicle, term life insurance regardless of face value, whole life insurance if the total face value is $1,500 or less, burial plots and headstones, prepaid irrevocable burial plans plus up to $1,500 in designated burial funds, and business property actively used in a trade or business that you can document. Countable assets include bank accounts, stocks, bonds, certificates of deposit, and additional vehicles. Four Ways to Send It Back

  • By mail to your county social services office. The return address prints on the form. Attach copies of your supporting documents, not originals.
  • In person at your local county office.

Keep a copy of whatever you send. Photograph the pages before sealing an envelope, or use certified mail. Disputes over whether a form was received do happen, and proof on your end resolves them fast.

After You Submit

A county caseworker reviews the packet and either processes the renewal or flags it as incomplete.6CalSAWS. Ignore the MC 355 and your coverage can be discontinued even if your original renewal was in on time.

If You Missed the Deadline

A missed deadline is not the end of your Medi-Cal. California gives you a 90-day cure period after your coverage is discontinued. Reinstatement is a safety net, not a plan. Between the discontinuance date and the day the county restores your case, medical services you receive may not be covered and providers can bill you directly. Meeting the original deadline is worth the effort.

If Your Renewal Is Denied or Reduced

If the outcome is a denial, a move from full-scope to restricted coverage, or a higher share of cost, you can request a state fair hearing. The standard deadline is 90 days from the date on the notice of action.
