Missouri Form 570: Historic License Plates Application Guide

Missouri Form 570 is the one-page application you use to put historic license plates on a collector vehicle or trailer that is at least 25 years old. You fill it out, attach a personal property tax receipt, proof of insurance, and a copy of your title, and send it with the fee to the Missouri Department of Revenue’s central office in Jefferson City. For a standard historic vehicle plate, the total comes to $34.25 and the registration is permanent.¹

Who Can Use Form 570

Eligibility is narrow, and you swear to it on the form itself. The vehicle — passenger car, truck, motorcycle, or RV — must be at least 25 years old based on model year, owned solely as a collector’s item, and used for exhibition and educational purposes.² For applications filed in 2026, that means a model year of 2001 or earlier.

Certain trailers qualify too. A camping trailer with collapsible side walls, or a fifth-wheel trailer used for recreational travel, is eligible if it meets the 25-year threshold.³

The vehicle or trailer must already be titled and registered in Missouri in your name before you apply.¹ Form 570 adds historic plates to an existing registration; it does not title the vehicle for you.

You are signing a sworn statement that all of this is true. A false entry can result in fines, imprisonment, or both.³

Pick a Plate Type Before You Start

The form gives you four choices, and the box you check determines your fee and whether you ever renew.

The Historic Vehicle option is the standard one for cars, trucks, motorcycles, and RVs. Register once, pay once, never renew.

Historic Trailer covers eligible camping and fifth-wheel trailers. Also permanent, but the fees run higher.

Personalized Historic lets you pick a custom configuration of up to six characters. Unlike the others, these plates expire and must be renewed on an annual or biennial cycle.¹

Year of Manufacture lets you display an original-era plate from the vehicle’s model year. You have to submit a photo of the plate with your application.

Filling Out the Form

Form 570 is a single page. Download the current version (revised September 2025) from dor.mo.gov, or pick one up at a license office.³

Owner Information

Enter your name exactly as it appears on the title. Add your driver’s license number, or FEIN if the vehicle is titled to a business. Fill in the street address, county, city, state, ZIP, and a phone number the Department can use if there is a problem.

Vehicle or Trailer Description

Copy the year, make, body style, and kind of vehicle from your title. Transcribe the VIN and title number carefully; a single wrong digit will bounce the application back.

Plate Type Selection

Check one box. For Personalized Historic, list up to six configuration choices ranked by preference, with a short description of what each one means. For Year of Manufacture, fill in the plate number and the year printed on the plate.

Affirmation and Signature

Two sworn statements sit at the bottom. The first affirms you maintain financial responsibility on every vehicle you own or operate. The second affirms the vehicle is over 25 years old, held solely as a collector’s item, and used for exhibition and education. Sign, print, and date. Both carry criminal weight if false.

What to Send With the Form

The application by itself is not enough. Include all of the following:¹

A personal property tax receipt or non-assessment statement from your county of residence (or the City of St. Louis) for the previous year. Original or copy.
A current insurance identification card showing the year, make, VIN, and expiration date. Original or copy. Trailers are exempt.
A copy of the title or registration receipt showing Missouri title in your name.
A clear photograph of the plate, if you are applying for Year of Manufacture.
Payment for the applicable fees.

What It Costs

Fees depend on the plate type you chose.¹

Historic Vehicle: $25.25 registration plus $9 processing. Total $34.25.
Historic Trailer: $52.50 registration plus $45 processing. Total $97.50.
Personalized Historic: $25.25 registration, $15 personalized plate fee, and $9 processing. Total $49.25 at first application, with the $15 and $9 due again each renewal.
Year of Manufacture (vehicle): $25.25 plus $9 processing. Total $34.25.
Year of Manufacture (trailer): $52.50 plus $45 processing. Total $97.50.

For every option except Personalized Historic, the registration fee is a one-time charge. The plates cannot be transferred to a different vehicle; if you sell the car, the plates go with it or become invalid.

Where to Send It

Historic plate applications do not go to a local license office. They go to the Department of Revenue’s central office in Jefferson City, by mail or in person.¹

By mail, address the envelope to the Motor Vehicle Bureau, PO Box 2046, Jefferson City, MO 65105-0100. In person, the office is at 301 West High Street, Jefferson City, MO 65101, Room 370.⁴ A trackable mailing method gives you proof of delivery if anything goes missing.

What You Are Agreeing To

Historic plates come with driving limits that the affirmation on Form 570 binds you to. Missouri law permits three uses:²

Exhibitions, antique auto tours, shows, and educational events, with no mileage limit.
Repair trips of up to 100 miles from the vehicle’s location.
Personal use, capped at 1,000 miles per calendar year.

You have to keep a mileage log for personal-use miles and carry it in the vehicle whenever it is on a state road. The Department provides Form 4012, a free PDF, for logging the date, purpose, destination, origin, and miles for each trip.⁵ Without the log in the vehicle, you cannot prove you are under the cap if a trooper asks. Violations can lead to penalties and revocation of the historic plates.²

The trade-off works in your favor on inspections. A vehicle with historic registration is exempt from both Missouri’s safety inspection and emissions inspection requirements, before you apply and for as long as the plates stay active.⁶

On renewal, standard Historic Vehicle, Historic Trailer, and Year of Manufacture plates are permanent and never expire. Only Personalized Historic plates renew, on an annual or biennial cycle, with the $15 personalized fee and $9 processing due each time.¹ Missing a renewal means losing the custom configuration, so put the expiration date somewhere you will see it.
