Missouri Lottery Winner Claim Form: Deadlines, Filing, and Payout

Any Missouri Lottery prize over $600 has to be collected using the Missouri Lottery Winner Claim Form. You download the one-page form from the Lottery’s website or pick one up at a Lottery office, fill it out, and submit it together with your signed original ticket, a government-issued photo ID, and a completed IRS Form W-9. You can mail the package to Jefferson City or bring it to a scheduled appointment at one of four Lottery offices. Prizes of $600 or less don’t need the form at all; a licensed retailer can cash those on the spot.

Act Before the 180-Day Deadline

Draw game tickets expire 180 days after the drawing date of the last winning play on the ticket. Scratchers expire 180 days after the official game ending date.1Missouri Lottery. Winning Ticket Tips Miss that window and the prize is gone. The Lottery posts an expired-Scratchers list on its site so you can check whether a game has already closed.

Sign the ticket as soon as you realize it’s a winner, and print your name, address, and phone number on the back. A signed ticket is the only proof of ownership the Lottery recognizes; an unsigned ticket can be claimed by whoever holds it.2Missouri Lottery. Claiming Prizes

What to Gather Before You Start

Three items go with the form:

The Lottery is not liable for lost, stolen, or damaged tickets, so protect the original the way you would cash.

Filling Out the Form

The Winner Claim Form is a single page and covers both Scratchers and draw game prizes. You can download it as a PDF from the Missouri Lottery website or pick up a paper copy at any Lottery office.4Missouri Lottery. Missouri Lottery Winner Claim Form

Enter your full legal name, current mailing address, phone number, and Social Security number or taxpayer identification number in the spaces provided. Fill in the ticket details next: the game name and the ticket’s serial number. Staple the original ticket to the top right corner of the form. Check the serial number on the form against the one on the ticket before you submit; a mismatch slows processing.

Two fields near the bottom matter if more than one person is claiming the prize. Section 13 asks how many people are claiming. Section 14 asks whether the prize should be split equally; if the split is unequal, each claimant writes in the specific dollar amount they are claiming.4Missouri Lottery. Submitting the Claim

You have two ways to turn the paperwork in.

In Person

In-person claims for prizes over $600 are handled by appointment only at the Jefferson City headquarters and the Springfield, St. Louis, and Kansas City regional offices. Call (573) 751-4050 and select Option 9, or use the online scheduling tool on the Lottery’s website.2Missouri Lottery. Powerball and Mega Millions jackpots, along with top-tier Cash4Life prizes, must be claimed at the Jefferson City headquarters. Regional offices cannot process them.2Missouri Lottery. Claiming Prizes

Claiming as a Group

When two or more people share a winning ticket, each person has to be identified on a claim form, and each needs their own completed IRS Form W-9. For prizes where federal reporting kicks in, the group representative who presents the ticket also fills out IRS Form 5754. The payer uses Form 5754 to prepare individual W-2G forms so each person’s share is reported to the IRS separately.5Internal Revenue Service. Settle the ownership split before you file. Sections 13 and 14 on the claim form are what the Lottery uses to issue separate tax documents based on the percentages you declare.4Missouri Lottery. Missouri Lottery Winner Claim Form

Jackpot Winners Have 60 Days to Elect Cash

If you’ve won a jackpot in Powerball, Mega Millions, MO Millions, or Cash4Life, you have exactly 60 days from the date you file the claim to choose the cash (lump sum) option. Miss that window and the prize automatically pays out in annual installments: 30 annual payments for Powerball, Mega Millions, and MO Millions, or annual payments for life for Cash4Life.6Missouri Lottery. If You Win a Jackpot The cash option is roughly half the advertised jackpot amount.

When You’ll Be Paid

Timing depends on the prize size and the game:

  • Prizes up to $35,000 are paid the same day at an in-person appointment.2Missouri Lottery. Claiming Prizes
  • Prizes over $35,000 take about two weeks from the day you file.
  • MO Millions jackpots take roughly 10 to 14 business days.6Missouri Lottery. If You Win a Jackpot
  • Powerball, Mega Millions, and Cash4Life jackpots run about 14 to 21 business days, because multiple state lotteries are involved in verifying and funding the prize.6Missouri Lottery. If You Win a Jackpot

For prizes of $35,000 and above, the Lottery offers direct deposit. Submit a completed Direct Deposit form with the rest of your claim paperwork if you want the funds sent electronically.2Missouri Lottery. Claiming Prizes Mailed claims take longer because of postal transit on both ends.

What’s Withheld Before You See the Check

Federal law requires the Lottery to withhold 24 percent from any prize over $5,000.7Office of the Law Revision Counsel. 26 U.S. Code 3402 – Income Tax Collected at Source Missouri adds a 4 percent state withholding on any lottery prize of $600 or more.8Missouri Revisor of Statutes. Missouri Code 313.321 – State Lottery Fund, Established On a $10,000 prize, that’s $2,400 federal and $400 state before anything else comes out.

Before cutting the payment, the Lottery runs a debt set-off check. Under Missouri law, it can intercept prize money for delinquent child support, unpaid debts to county jails, and outstanding health-care debts owed to hospitals and providers.8Missouri Revisor of Statutes. Missouri Code 313.321 – State Lottery Fund, Established A separate statutory process handles delinquent state taxes and other debts owed to state agencies; if you owe, the department notifies you and the amount is deducted from your prize.9Missouri Revisor of Statutes. Missouri Code 143.784 – Debt Owed to State Agency, Setoff Procedure The check you receive is the prize minus federal and state withholding and any verified debts.

Your Name Stays Private by Default

Missouri law prohibits the Lottery from publishing the name, address, or other identifying information of any winner without written consent.10Missouri Revisor of Statutes. Missouri Code 313.303 – Prohibits Publishing Names and Other Information Concerning Lottery Winners The statute goes further: the Lottery cannot even offer you a consent form unless you ask for one, and if you do ask, the form has to state in bold 14-point font that signing is not required to collect your winnings. Anonymity is the default; you’d have to opt in to publicity.
