Who Can Claim the Reimbursement

  • You had active MO HealthNet coverage on the date of the trip.
  • The appointment was for a service covered by MO HealthNet.
  • You had no free ride available. If a relative, volunteer, or the medical facility itself could have driven you at no cost, you are not eligible.

Reimbursement also has to be the least costly option that fits your medical condition. Filling Out the Log

Each line represents one trip. You can record multiple trips on a single form; keep each on its own line, with your identifying information, your driver’s name, and the appointment details.

If Your Trip Is Longer Than the State’s Standard Distance
