To keep a Montana commercial driver’s license active, you need a current Medical Examiner’s Certificate on file with the Motor Vehicle Division, issued by a doctor listed on the FMCSA’s National Registry after you pass a DOT physical. Montana’s CDL medical card requirements also include a self-certification telling the state what kind of driving you do, which determines whether a medical card applies to you at all. Let the certificate lapse and the Motor Vehicle Division will downgrade your CDL within 10 days.1Montana State Legislature. Montana Code 61-5-141 – Self-Certification of Operation Status
Who Needs a Medical Card in Montana
Before Montana will issue or renew a CDL, you have to tell the Motor Vehicle Division which of four federal categories your driving falls into. That self-certification controls whether a medical card is required.1Montana State Legislature. Montana Code 61-5-141 – Self-Certification of Operation Status2eCFR. 49 CFR 383.71 – Driver Application and Certification Procedures
- Non-excepted interstate: you cross state lines and must meet all federal physical qualification standards. Medical card required.
- Excepted interstate: you cross state lines but drive only in categories exempt from the medical rules, such as certain federal and military vehicles. No medical card.
- Non-excepted intrastate: you drive only within Montana and must meet state driver qualification requirements. Medical certificate or a medical statement prescribed by department rule required.
- Excepted intrastate: you drive only within Montana in operations the state exempts. No medical card.
If your work changes category, update the Motor Vehicle Division. Driving in a non-excepted category without a current certificate on file is what triggers the downgrade.
The DOT Physical and the Forms You’ll Get
The physical has to be done by a certified medical examiner on the FMCSA’s National Registry, which you can search by city, state, or zip code.3Federal Motor Carrier Safety Administration. National Registry of Certified Medical Examiners A regular family doctor who hasn’t passed FMCSA training can’t do it.
Two forms come out of the exam. The Medical Examination Report, Form MCSA-5875, documents your history and the examiner’s findings.4Federal Motor Carrier Safety Administration. Medical Examination Report Form MCSA-5875 If you pass, you get the Medical Examiner’s Certificate, Form MCSA-5876, which is your medical card.5Federal Motor Carrier Safety Administration. Medical Examiners Certificate Form MCSA-5876 You then submit it to the Montana Motor Vehicle Division.1Montana State Legislature. Montana Code 61-5-141 – Self-Certification of Operation Status The examiner keeps copies for at least three years.
Expect to pay $75 to $150 out of pocket. Insurance rarely covers it.
What the Examiner Checks, and Where Drivers Trip Up
The federal physical qualification standards in 49 CFR 391.41 set the baseline. A few areas account for most of the surprise denials and shortened certifications.
Blood Pressure
This is where drivers most often lose months of eligibility on a single reading. The FMCSA ties your certification length to your hypertension stage:6Federal Motor Carrier Safety Administration. 49 CFR 391.41(b)(6) – Driver Safety and Health Medical Requirements
- Stage 1 (140–159 / 90–99): one-year certificate. The next exam must come in at or below 140/90 to earn another year.
- Stage 2 (160–179 / 100–109): a one-time three-month certificate to start or adjust treatment. Bring your reading below 140/90 inside that window and you can get a one-year certificate.
- Stage 3 (180+ / 110+): immediate disqualification. Once treatment brings you under 140/90, certification runs in six-month intervals.
If your readings have been climbing, address it before exam day, not at the clinic.
Vision and Hearing
You need at least 20/40 distant acuity in each eye and binocularly, a 70-degree horizontal field in each eye, and the ability to recognize standard traffic signal colors.7eCFR. 49 CFR 391.41 – Physical Qualifications for Drivers8Federal Motor Carrier Safety Administration. General Vision Exemption Package9eCFR. 49 CFR 391.45 – Persons Who Must Be Medically Examined and Certified
For hearing, you must perceive a forced whisper at five feet in your better ear, with or without a hearing aid. Audiometric testing is an alternative, with a 40-decibel ceiling on average loss at 500, 1,000, and 2,000 Hz in the better ear.7eCFR. 49 CFR 391.41 – Physical Qualifications for Drivers Drivers who can’t meet the standard can apply for a federal hearing exemption, but the process includes a 30-day public comment period and takes months.10Federal Motor Carrier Safety Administration. Federal Hearing Exemption Application
Insulin-Treated Diabetes and Seizure History
Insulin-treated diabetes is no longer an automatic disqualifier. The clinician who manages your insulin must complete the Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870) before each exam, and the medical examiner then decides whether you’re safe to drive.11eCFR. 49 CFR 391.46 – Physical Qualification Standards for an Individual With Diabetes Mellitus Treated With Insulin for Control Certification isn’t available if your regimen isn’t stable or if you have severe diabetic retinopathy, and re-examination is annual rather than every two years.
Epilepsy or any condition likely to cause loss of consciousness is disqualifying under the base rule. Drivers who have been seizure-free for at least eight years may apply for a federal seizure exemption; if you stopped anti-seizure medication, the eight years runs from the date you discontinued it.12Federal Motor Carrier Safety Administration. Federal Seizure Exemption Application
Medications, Including Marijuana
Bring a full list of everything you take, including supplements and over-the-counter products. Any Schedule I substance, amphetamine, narcotic, or other habit-forming drug makes you medically unqualified.7eCFR. 49 CFR 391.41 – Physical Qualifications for Drivers For Schedule II–V prescriptions, your prescribing doctor can write a statement that the medication won’t impair your driving, though the examiner can still deny certification.13Federal Motor Carrier Safety Administration. What Medications Disqualify a CMV Driver
Two substances catch drivers off guard. Marijuana is prohibited for all safety-sensitive DOT employees regardless of state law. As of late 2025, the DOT has confirmed that marijuana remains a Schedule I substance and testing requirements are unchanged.14U.S. Department of Transportation. DOT Notice on Testing for Marijuana Methadone is the other: the prescription exception does not apply, so a driver on methadone is unqualified even with a doctor’s approval. Anti-seizure medications taken to prevent seizures are also automatically disqualifying regardless of the prescriber’s opinion.13Federal Motor Carrier Safety Administration. How Long the Card Lasts and When to Renew
A standard medical certificate is valid for up to 24 months.9eCFR. 49 CFR 391.45 – Persons Who Must Be Medically Examined and Certified Several conditions shorten it:
- Insulin-treated diabetes: annual.
- Alternative vision standard: annual.
- Stage 1 hypertension: one year.
- Stage 2 hypertension: one-time three-month certificate.
- Stage 3 hypertension treated to below 140/90: six-month intervals.
Schedule your renewal exam well before the expiration date. Under Montana law, a driver certified for non-excepted interstate commerce must submit a new certificate before the current one expires.1Montana State Legislature. Montana Code 61-5-141 – Self-Certification of Operation Status If your driving category has shifted since your last certification, update that at the same time.
If Your Medical Certificate Lapses
When no new certificate is on file, the Montana Motor Vehicle Division has 10 days to downgrade your CDL.1Montana State Legislature. Montana Code 61-5-141 – Self-Certification of Operation Status A downgraded license loses its commercial privileges. You can still drive personal vehicles, but you cannot legally operate a commercial motor vehicle until you fix the problem, and most employers will pull you off the road the moment your certification lapses.
Reinstatement is possible within the original term of the downgraded license. You restore the CDL by submitting a current medical certificate, or by changing your self-certification to an excepted category that doesn’t require one.1Montana State Legislature. Falsifying Medical Information
Lying on your medical forms or CDL application is a crime in Montana. Under Montana Code 61-5-302, using a false name, making a knowingly false statement, concealing a material fact, or committing fraud in the application process is a misdemeanor, and the Motor Vehicle Division can suspend or revoke the license of anyone who commits fraudulent use.15Montana State Legislature. Montana Code 61-5-302 – Unlawful Use of License or Identification Card Medical examiners also transmit exam results to the FMCSA’s National Registry, so discrepancies between what you report and what the examiner finds end up in a federal database and can disqualify you from commercial driving in every state.
