NC CDL Renewal Requirements: Documents, Fees, and Medical Certification

Renewing a commercial driver license in North Carolina means an in-person visit to a driver license office every five years with proof of identity, your Social Security number, two documents showing your North Carolina address, a completed self-certification form, and the applicable fees. The NC CDL renewal requirements cannot be met online or by mail.¹ One change catches many drivers off guard: as of June 28, 2025, the DMV no longer accepts a Medical Examiner’s Certificate handed over the counter. Your medical examiner has to upload it electronically before you arrive.²

Documents To Bring

The document list mirrors what you needed for an initial CDL. Bring proof of identity such as a valid U.S. passport, birth certificate, or permanent resident card; your Social Security card or an SSA document showing your full number; and two separate documents proving your current North Carolina physical address, such as utility bills, bank statements, voter registration cards, or lease agreements.

If you are not a U.S. citizen, federal rules require proof of lawful presence, generally an unexpired Employment Authorization Document or an unexpired foreign passport with an approved I-94.³ Your CDL cannot be issued for longer than the validity of those immigration documents, so plan the renewal timeline around them.

Medical Self-Certification and the Electronic Certificate

Every CDL holder must self-certify to a type of commercial operation at each renewal using form CDL-MED-1.² You pick one of four categories:

Non-Excepted Interstate. You drive across state lines and must meet all federal medical standards. A DOT medical card is required. Most long-haul drivers fall here.
Excepted Interstate. You drive across state lines but qualify for a federal exemption from medical certification.
Non-Excepted Intrastate. You drive only within North Carolina and must meet state medical requirements. A DOT medical card is required, and the license carries a “K” restriction.
Excepted Intrastate. You drive only within North Carolina and are exempt from medical certification. The “K” restriction applies.

Download and complete CDL-MED-1 before your appointment.⁴

If your category requires a medical card, the certificate has to come from a provider on FMCSA’s National Registry of Certified Medical Examiners.⁵ Since June 28, 2025, the examiner submits it electronically through FMCSA’s National Registry Integration Initiative portal. Paper certificates handed to the DMV are no longer accepted.² Call the examiner’s office before your appointment and confirm the upload has posted. If the DMV cannot see a “certified” status in the federal system when you arrive, the visit is wasted.

Drivers who miss the medical requirement get 60 days to come into compliance. After that, the state downgrades or restricts commercial driving privileges.⁶

Hazmat Endorsement: Start Early

Renewing an H or X endorsement adds a federal step. Under 49 CFR Part 1572, North Carolina cannot renew the endorsement without a TSA security threat assessment clearance.⁷ The state cannot speed up TSA processing, so begin three to four months before your expiration.

Schedule fingerprinting through TSA’s enrollment portal at tsaenrollmentbyidemia.tsa.dhs.gov and pay the $85.25 federal screening fee.⁸ Once TSA clears you, it sends a Determination of No Security Threat to the state. Without that clearance, the DMV strips the hazmat endorsement from your renewed license.

You also retake the written hazmat knowledge test at each renewal.³ Study materials are in the hazardous materials section of the North Carolina CDL manual.

Drug and Alcohol Clearinghouse Check

Since November 2024, North Carolina queries the FMCSA Drug and Alcohol Clearinghouse before renewing any CDL. If your record shows a prohibited status, renewal is denied until the prohibition is resolved.³ The DMV runs the check automatically. If you have gone through a return-to-duty process, verify your Clearinghouse status is current before making the trip.

Renewal Fees

A North Carolina commercial license costs $25.50 per year. A five-year renewal totals $127.50. Each endorsement adds $5.00 per year, or $25.00 over the five-year term.⁹ Two endorsements bring the total to $177.50.

A hazmat endorsement adds the $85.25 TSA background check fee on top of the state charges. TSA also offers a reduced rate of $41.00 for some applicants.⁸ Driver license offices accept common payment methods; CDL renewals themselves have to be handled in person, not through MyNCDMV.

The In-Person Appointment

Book through the NCDOT’s SkipTheLine system at skiptheline.ncdot.gov. Some CDL-specific services may require calling 919-715-7000.¹⁰

At the visit, expect a vision screening and a review of your documents. The examiner checks your driving record for disqualifications and confirms your medical status shows certified in the federal system. You surrender the expiring license and leave with a temporary paper credential; the permanent card is mailed within a few weeks.

Not meeting 20/40 acuity in each eye during the medical exam is not an automatic disqualification. Since March 2022, FMCSA replaced its old vision exemption program with an alternative vision standard. A driver who cannot meet the standard with the worse eye is evaluated by an ophthalmologist or optometrist using the Vision Evaluation Report (Form MCSA-5871), and the medical examiner then decides qualification under the updated rule.¹¹

If Your CDL Expires

An expired CDL means you cannot legally operate a commercial vehicle. North Carolina offers no grace period for CDLs, and the state’s recent moratorium on driver license expirations specifically excluded them.¹² The day after expiration, your commercial authority is gone.

A CDL also cannot be renewed if your license is suspended, revoked, or cancelled in any state, or if you are currently disqualified from operating a commercial vehicle.⁶

School Bus Drivers Renew Every Three Years

If your CDL carries both the passenger (P) and school bus (S) endorsements and you are certified to drive a school bus in North Carolina, the license expires on your birthday in the third year after issuance rather than the fifth.¹³ It’s easy to assume the standard five-year cycle applies when it doesn’t, so check the date on the card.

Entry-Level Driver Training Doesn’t Apply to Renewals

The federal Entry-Level Driver Training rule that took effect in February 2022 does not affect renewals. ELDT applies only when you obtain a Class A or B CDL for the first time, upgrade from Class B to Class A, or add a school bus, passenger, or hazmat endorsement for the first time.¹⁴ Renewing an existing CDL with the same class and endorsements you already hold triggers no ELDT obligation.
