The NC Medicaid spend down is a way to qualify for Medicaid when your income or assets are too high for the regular limits. It works in two parts. If your income is too high, you can subtract your medical bills from it until you meet a threshold, and Medicaid then covers care for the rest of a set period. If your assets are too high and you need long-term care, you reduce countable resources down to the asset limit by spending them on allowed items. How the Income Spend Down Is Calculated
Standard Medicaid for aged, blind, and disabled adults in North Carolina generally requires monthly income at or below 100% of the federal poverty level, roughly $990 for an individual or $1,335 for a household of two.1Buncombe County Government. Your deductible is based on the Medically Needy Income Limit, a much lower figure: $242 per month for an individual and $317 for a couple.2NC DHHS. Basic Medicaid Eligibility Chart The state subtracts the MNIL from your countable monthly income, then multiplies the difference by six for a standard six-month certification period. That total is what you have to spend on medical care before Medicaid picks up the rest.
Here’s how the math plays out. Suppose your countable income is $1,200 a month. Subtract $242 and you have $958 in excess income. Multiplied by six, the deductible is $5,748. Once you incur medical bills equal to that amount, Medicaid coverage begins and continues through the end of the six-month period.3LawHelp North Carolina. Picking the Budget Period
You aren’t required to use a six-month prospective window. The deductible can be calculated for a retroactive period of one, two, or three months before you apply, or for a six-month prospective period starting with the application month.1Buncombe County Government. Medicaid for Age 65+, Older, Blind, or Disabled A shorter retroactive period gives you a smaller deductible to hit, but also a shorter window of coverage once you clear it.
Timing matters within the six-month cycle too. Because coverage runs from the day you meet the deductible through the end of the certification period, holding a large bill until the start of a new cycle can produce more months of active coverage than submitting it near the end of an old one.3LawHelp North Carolina. What Medical Bills Count
A broad range of costs can be applied to the deductible:
- Hospital and clinic charges
- Prescription and over-the-counter drugs
- Medical supplies and equipment
- Transportation to appointments, including ambulance, bus, and taxi fares

One point that trips people up: North Carolina counts both paid and unpaid bills. You don’t have to have written a check. Incurring the bill is enough. Only the portion you personally owe counts; anything an insurer already paid doesn’t. Older Bills
Unpaid bills from before the current certification period can be used too. The service or payment date must fall within 24 months before your application month, the provider must still be billing for it (not written off), and it can’t have already been used to satisfy an earlier deductible. If you paid a medical bill using a credit card or personal loan, the outstanding balance (minus finance charges) can be counted as of day one of the certification period.7NC DHHS. Order of Application
Expenses hit the deductible in a set order. Unpaid balances from before the period apply first, on day one. Then charges from within the period apply in chronological order, with non-covered services applied before covered ones. An inpatient hospital admission without Medicare Part A coverage automatically meets the deductible, whatever the total charges come to.7NC DHHS. MA-2360 Medicaid Deductible Policy
Asset Spend Down for Long-Term Care
If you’re applying for Medicaid to cover nursing home care, income is only part of the test. North Carolina caps countable assets at $2,000 for an individual and $3,000 for a married couple.8NC Estate Planning. Medicaid Spenddown Anything above that has to be spent down before you qualify.
Countable assets include cash, bank accounts, retirement accounts, stocks and bonds, second homes, additional vehicles beyond one, and life insurance with cash value.8NC Estate Planning. Medicaid Spenddown Exempt from the limit:
- Your primary residence, if you intend to return home or a spouse or dependent relative lives there
- One vehicle, at any value
- Personal belongings, clothing, furniture, and household goods
- Term life insurance, regardless of face value

The usual way to bring countable assets down is to convert them into exempt ones or pay off legitimate obligations. That includes paying off a mortgage, buying a pre-paid irrevocable funeral plan, replacing a vehicle, making home repairs or improvements, and paying down debt. Purchases must be at fair market value.
The Five-Year Look-Back
North Carolina reviews the 60 months before your application for asset transfers.10NC DHHS. MA-2240 Transfer of Assets Policy If you gave anything away or sold it for less than it was worth during that window, the state imposes a penalty period during which Medicaid will not pay for institutional services, including nursing facilities and home-based alternatives like the Community Alternatives Program.
The penalty is based on the total uncompensated value of transfers during the look-back: the difference between what the asset was worth and what you got for it. Multiple transfers are added together and treated as a single transfer to calculate one penalty period.10NC DHHS. MA-2240 Transfer of Assets Policy Once the look-back date is set, it doesn’t move.
Annuities have their own trap. For any annuity bought or changed on or after November 1, 2007, North Carolina must be named as the remainder beneficiary. If it isn’t, the full purchase price counts as an uncompensated transfer and triggers a penalty.10NC DHHS. MA-2240 Transfer of Assets Policy
Protections When One Spouse Stays Home
If one spouse enters a nursing home and the other continues living at home, the Community Spouse Resource Allowance protects part of the couple’s combined countable assets. The CSRA is set based on the couple’s total countable assets on the first day of the institutionalized spouse’s continuous care, on this tiered scale:11NC DHHS. - $32,532 or less: all assets protected
- $32,532 to $65,064: $32,532 protected
- $65,064 to $325,320: half of total assets protected
- Over $325,320: $162,660 protected
Assets above the CSRA plus the individual reserve limit have to be spent down before the institutionalized spouse qualifies. A protection period then runs through the sixth month of the certification period, and the protected assets should be moved into the community spouse’s name within that window. Income is handled under a “name on the check” rule. Only income in the applicant’s name counts toward Medicaid eligibility, and the community spouse’s own income is disregarded.12The Elder Law Firm. Treatment of Income for Medicaid
The community spouse may also draw a Monthly Maintenance Needs Allowance from the institutionalized spouse’s income. As of mid-2025, the federal minimum is $2,643.75 per month, with a maximum of $3,948.13Medicaid.gov. After You Qualify: Patient Liability
Once you’re eligible for Medicaid in a nursing home, most of your monthly income goes toward the cost of care. This is your patient liability. You keep a small personal needs allowance: $30 a month for an individual, $60 for a married couple when both are institutionalized. Certain deductions come out before patient liability is calculated, including uncovered medical costs, Medicare supplemental premiums, and any spousal income allowance.12The Elder Law Firm. Who Doesn’t Need the Spend Down
Some people qualify without going through this process at all. If you receive Supplemental Security Income, you get Medicaid automatically. If you can’t meet a deductible but need help with Medicare costs, Medicare Savings Programs can pay Medicare premiums, deductibles, and coinsurance at various levels based on income.1Buncombe County Government.
