The Nevada Business Registration form, TAX-F006, is filed with the Nevada Department of Taxation to open your state tax accounts: Sales and Use Tax, Modified Business Tax, Commerce Tax, and any others your activities trigger. You can submit it online at mynvtax.nv.gov or mail it to the Department of Taxation at 3850 Arrowhead Drive, Carson City, NV 89706.1Nevada Department of Taxation. Nevada Business Registration Form
One thing to settle before you start: TAX-F006 is not the State Business License. The license comes from the Secretary of State through the SilverFlume portal under NRS Chapter 76.2Nevada Legislature. Nevada Code 76 – State Business Licenses TAX-F006 goes to a different agency entirely, and the Secretary of State’s office does not pass your information along to the Department of Taxation. You have to register with each separately.3Nevada Secretary of State. State Business License – FAQ
What to Have Ready Before You Open the Form
Fill in the blanks ahead of time. Hunting for information mid-application is where errors creep in, and omissions are the single most common reason the Department sends an application back.1Nevada Department of Taxation. Nevada Business Registration Form
- Your Federal Employer Identification Number. Get it from the IRS at irs.gov or by calling 1-800-829-4933 before you start the Nevada form; a pending FEIN application will stall your registration. It goes in Section 6.
- Your six-digit NAICS code. Look it up at census.gov/naics rather than guessing. The code goes in Section 19 and directly affects your Commerce Tax rate and how the Department categorizes your filings.
- Entity formation details: state and date of incorporation or organization (Section 7), and the exact legal name as filed with the Secretary of State (Section 8). Even small discrepancies, like an ampersand where your Secretary of State filing uses “and,” can cause rejection.
- Ownership details for every owner, partner, officer, manager, or member: full legal name, home address, Social Security Number or ITIN, date of birth, title, and ownership percentage (Section 14).
- Your ESD/UI number from the Department of Employment, Training and Rehabilitation if you have employees (Section 17).
- Realistic monthly revenue estimates: total Nevada monthly receipts (Section 21) and estimated taxable receipts (Section 22). These numbers set your reporting cycle and any Sales Tax security deposit.
Filling Out the Sections That Trip People Up
Business Identity and Addresses
Sections 8 through 13 establish who you are and where you operate. Enter your legal entity name exactly as filed with the Secretary of State, and if you use a DBA, list it alongside in Section 8. The form asks for three different addresses: the physical location of your Nevada operations (Section 10), a mailing address for licenses and correspondence (Section 11), and the location where you keep your business records (Section 13). Fill in all three even when they overlap, because the Department uses separate addresses for different types of notices.1Nevada Department of Taxation. Nevada Business Registration Form
Business Activities (Section 18)
Section 18 is a checklist of activities: retail (new and used), wholesale, manufacturing, construction, leasing, tobacco, live entertainment, cannabis, liquor retail, marketplace facilitator or seller, and more. Check every box that applies. The Department uses this list to decide which tax accounts to open for you, so an unchecked activity is a tax account you won’t have, and often one you don’t realize is missing until a delinquency notice arrives. If you sell retail goods and also lease equipment, both boxes get checked.1Nevada Department of Taxation. Nevada Business Registration Form
Revenue Estimates (Sections 21 and 22)
These are not formalities. The Department assigns your filing cycle (monthly, quarterly, or annual) based on the numbers you put here, and the figures feed into the security deposit calculation for Sales Tax permits. Overestimating locks you into more frequent filing than you need. Underestimating can trigger penalties when your actual collections run higher. Use realistic numbers from your business plan or comparable businesses in your industry.
Which Tax Accounts the Form Opens
TAX-F006 is a single form that creates multiple tax accounts depending on which activities you checked. Knowing what each one is helps you fill out Section 18 correctly.
Sales and Use Tax
If you sell tangible personal property at retail in Nevada, you need a Sales Tax permit. If you buy goods from out of state for your own use in Nevada rather than for resale, you need a Use Tax account. The Secretary of State’s office directs business owners to contact the Department of Taxation to determine whether Use Tax registration applies to them.3Nevada Secretary of State. State Business License – FAQ The rate you collect depends on the county where the sale occurs.
Modified Business Tax
If you pay wages to Nevada employees, you owe Modified Business Tax each quarter. For general businesses, the rate is 1.475% on the portion of quarterly wages that exceeds $50,000.4Nevada Legislature. Nevada Code 363B – Business Tax Nonprofits, Indian tribes, political subdivisions, and domestic service employers are exempt.5Nevada Department of Taxation. Modified Business Tax
Commerce Tax
Commerce Tax applies only to businesses with Nevada gross revenue above $4,000,000 in a taxable year. Below that, no return is required. Above it, you subtract $4,000,000 from your gross revenue and apply the rate assigned to your NAICS-based industry category.6Nevada Legislature. Nevada Code 363C – Commerce Tax This is the point where your NAICS code selection has a direct dollar impact.
Security Deposit on Sales Tax Permits
New businesses applying for a Sales Tax permit should plan for a security deposit. The amount is calculated as twice your estimated average quarterly tax if you file quarterly, three times the estimated monthly tax if you file monthly, or four times the estimated annual tax if you file annually.7Nevada Legislature. Nevada Code 372 – Sales and Use Taxes No deposit is required if the calculated amount comes to $1,000 or less.8Cornell Law Institute. Nevada Admin Code 372.825 – Security Required for Payment
The Department accepts cash, surety bonds from insurance companies, and irrevocable letters of credit from Nevada-based banks or credit unions.8Cornell Law Institute. Nevada Admin Code 372.825 – Security Required for Payment Corporate taxpayers can ask the Tax Commission for a waiver by substituting personal surety from at least two principals (or one if there’s only one principal), provided they have no more than one delinquency, late return, or returned check in the preceding 36 months.

Mistakes That Stall Applications
The Department flags incomplete applications more often than wrong ones. The recurring issues:
- Submitting before your FEIN comes through from the IRS. Wait until you have the number.
- Picking a NAICS code that doesn’t actually match what your business does. It skews your Commerce Tax rate and your payroll categorization. Use the Census Bureau’s search tool rather than guessing.
- Leaving someone out of Section 14. Every owner, officer, manager, and member needs full details: SSN or ITIN, home address, date of birth, title, and ownership percentage. One missing partner triggers a follow-up that stalls the whole application.
- Skipping a box in Section 18 for an activity you actually do. Each unchecked activity is a tax account the Department won’t open for you.
- Entering an entity name that doesn’t exactly match your Secretary of State filing. Match the punctuation and spacing character for character.
Once the Department processes a complete application, you’ll receive your Sales Tax permit and any other account numbers your activities require. Keep the permit accessible at your place of business, and watch for the filing schedule the Department assigns, because returns are due on that cycle even in periods when you owe nothing.
