Who Can Still Use Form 629
An entity enters “default” the moment it misses its annual list deadline. If the default isn’t cured by the last day of the anniversary month the following year, the Secretary of State revokes the charter and forfeits the entity’s right to transact business in Nevada.3Nevada Legislature. Nevada Code 86 – Limited-Liability Companies – Section: NRS 86.274 A revoked entity cannot bring lawsuits in state court, and its property is held in trust by its managers or members.
There is a hard cutoff. Once a charter has been revoked for five consecutive years, the Secretary of State will not reinstate it.1Nevada Legislature. What Goes on Form 629
The form itself is short. You’ll enter the entity’s exact legal name as it appears in state records, its Nevada Business Identification Number (the NV Business ID or entity number), and the entity type. Different statutes govern each structure: NRS 78.180 for private corporations, NRS 86.276 for domestic LLCs, and NRS 86.5467 for foreign LLCs.1Nevada Legislature. The signature line matters. Form 629 includes a declaration under penalty of perjury that the reinstatement has been authorized either by a Nevada court or by the entity’s board of directors (for a corporation) or managers or managing members (for an LLC).1Nevada Legislature. The Companion Documents

- A completed and signed Initial/Annual List reflecting the people currently running the entity. For corporations, that means current officers and directors. For LLCs, it means current managers or managing members, not the lineup from when the entity was last in good standing.
- A State Business License Application covering the business license fee. If the entity qualifies for an exemption, submit a completed Declaration Page claiming it instead.
- A signed acceptance from the entity’s current registered agent, or, if you’re switching agents, a Statement of Change of Registered Agent signed by the new agent along with a $60 filing fee.
- A Name Consent form or supplemental Application of Reinstatement if another entity has claimed your business name during the revocation period.
Prepare every document before you submit. An incomplete package is the most common reason filings get returned.
What It Will Cost
Every reinstatement carries the $300 flat fee. The back fees and penalties on top are where the total climbs, and the amount depends on entity type and years missed.
Corporation
For each year of default, a Nevada corporation owes:

A small-share-structure corporation three years out of compliance owes roughly $300 + (3 × $150) + (3 × $75) + (3 × $500) + (3 × $100), which comes to $2,775. Larger share structures push that number well higher.
LLC
LLCs pay a lower business license fee, so the totals stay more predictable:

Three missed years for an LLC comes to $300 + (3 × $150) + (3 × $75) + (3 × $200) + (3 × $100), or $1,875.

After the Filing Is Accepted
The Secretary of State issues a Certificate of Reinstatement once the package clears. You can request the certificate as part of your filing for an additional issuance fee.1Nevada Legislature. Nevada Code 78 – Private Corporations – Section: NRS 78.180 It’s useful proof for banks, vendors, and courts that the entity is back in good standing.
Reinstatement relates back to the date the entity originally lost its right to do business, so legally the revocation is treated as though it never happened.4Nevada Legislature. Nevada Code 86 – Limited-Liability Companies – Section: NRS 86.276 Relation-back doesn’t erase everything that happened during the gap. Contracts signed while revoked can face enforceability questions, and lawsuits filed during the revocation may already have been dismissed. Why Filings Get Rejected
A few recurring problems account for most bounced applications:
