Renewing a New York medical license means renewing your biennial registration with the New York State Education Department’s Office of the Professions. The license itself is permanent; the registration costs $600 every two years, requires that several mandatory trainings be current, and must be completed before your existing registration expires to avoid late fees, professional misconduct exposure, or in a prolonged lapse, felony liability for unauthorized practice.
What You’re Actually Renewing
New York separates licensure from registration. Your license confirms you met the Education Law requirements to practice medicine and stays valid for your entire career unless the Board of Regents revokes, annuls, or suspends it. The Fee
The registration fee is $600. It combines a $570 base fee with a $30 surcharge that funds the state’s Professional Medical Conduct Account.3New York State Senate. Physicians who certify to the Department that they will practice only without compensation during the registration period are not required to pay the fee.3New York State Senate. How to Renew
Roughly four months before your registration expires, the Office of the Professions mails a Registration Renewal Document containing a seven-character PIN.4New York State Education Department. Online
Online renewal is faster and gives immediate confirmation. To use it, no more than five months can remain in your current registration period, or your registration must have lapsed less than four months ago; you also need a valid credit card and your PIN.2New York State Education Department. The application asks you to verify your name, license number, and the last four digits of your Social Security number. You’ll also answer questions about criminal convictions, professional misconduct charges, and institutional actions such as changes in hospital privileges. A “yes” answer requires a written explanation and supporting documentation, like certified court records or copies of disciplinary decisions, mailed separately to the Office of the Professions.2New York State Education Department. By Mail
You can still mail the paper Registration Renewal Document. The envelope should be postmarked before your registration expires, and certified mail gives you proof of timely submission. If your expiration date is close, the online system is safer.
Once payment clears and the application is approved, the Office of the Professions mails a new Registration Certificate. Trainings That Must Be Current
Several mandatory trainings sit alongside the registration. They run on their own timelines, which rarely line up with the two-year registration cycle, so tracking each one separately is essential.
Infection Control and Barrier Precautions
Public Health Law Section 239 requires every physician practicing in New York to complete an approved course in infection control and barrier precautions every four years. The training covers protocols for preventing transmission of HIV, hepatitis B, hepatitis C, and infections that could lead to sepsis.6New York State Department of Health. Child Abuse Identification and Reporting
Education Law Section 6507(3)(a) requires two hours of coursework on identifying and reporting child abuse and maltreatment, originally a one-time obligation completed at initial licensure.7New York State Senate. An amendment effective in 2024 added protocols for identifying abused or maltreated children with intellectual or developmental disabilities, and all mandated reporters must complete this updated training by November 17, 2026. Mandatory Prescriber Education
Physicians with a DEA registration who prescribe controlled substances must complete at least three hours of coursework in pain management, palliative care, and addiction every three years under Public Health Law Section 3309-a.9New York State Education Department. Federal MATE Act Training
The federal Mainstreaming Addiction Treatment (MATE) Act adds a one-time, eight-hour training obligation for DEA-registered practitioners covering treatment and management of patients with opioid and other substance use disorders. You attest to completion when applying for or renewing your DEA registration. Physician Profile Update
Public Health Law Section 2995-a requires you to update your Physician Profile in the Department of Health’s system within six months before your registration expires.12New York State Department of Health. Skipping it won’t necessarily block the registration from processing, but it is a separate legal obligation tied to the same cycle.
CME Is Not Required
New York does not require continuing medical education credits as a condition of registration renewal. This surprises physicians licensed in multiple states. The mandatory trainings above are specific courses, not CME-hour accumulation. Specialty boards and hospital credentialing committees may still require CME for their own purposes, but the state registration itself has no CME threshold.
If Your Registration Lapses
The consequences escalate with time.
First is a $10 late fee for each month the registration is overdue. The most serious risk is criminal. Under Education Law Section 6512, practicing any licensed profession without a current registration constitutes unauthorized practice, which is a Class E felony in New York.14New York State Education Department. Federal Renewals That Run Alongside
State registration is the main event, but three federal obligations move on their own schedules.
DEA Registration
Your DEA controlled substance registration renews independently. Since April 2022, all DEA renewals must be filed online, and the agency no longer mails paper reminders. Electronic notices go to the email address on file at 60, 45, 30, 15, and 5 days before expiration. Federal law prohibits handling controlled substances under an expired DEA registration at any time, regardless of whether you’ve submitted a renewal application. NPI Record Updates

Medicare Enrollment Revalidation
Physicians enrolled in Medicare must revalidate through PECOS every five years.
