If you are applying for a hoisting machine operator or rigger license, the LIC-62 is not the right form. The 90-Day Window

The exam date recorded on the LIC-62 cannot be more than 90 days before you submit it. If it is, the form is expired and you need a new appointment.1NYC Department of Buildings. LIC-62 Physical Examination Form A practical rule: book the physical no earlier than about 60 days before you plan to file, which leaves a buffer if the doctor’s office is slow to return the signed PDF or if the DOB kicks something back for a correction.

Download the blank form from the Department of Buildings website and fill in your applicant section (name, date of birth, Social Security number, address, license type, and current license number if renewing) before you go to the appointment. Bring the printed form with you. Your physician needs to understand what the license authorizes you to do, because the fitness determination is tied to the duties of your specific trade; the addendum on the form itself defines the standard for each listed category.

What the Physician Certifies

The LIC-62 does not spell out a required battery of clinical tests. It gives the doctor a functional standard and leaves the clinical choices to their judgment.   • Sudden loss of consciousness or incapacity
  • Impairment of awareness or concentration
  • Significant limitation of mobility that would prevent you from performing your licensed duties

The form also calls for a mental fitness evaluation covering cognitive function and emotional state as they relate to your license duties. There is no list of automatically disqualifying diagnoses. A well-managed chronic condition does not necessarily block certification; the question is whether, at the time of the exam, the condition creates a realistic risk of the impairments above.

The Three Outcome Boxes

After the exam, the physician checks one of three boxes:

  • Medically fit, with no restrictions.
  • Medically fit with accommodations. A separate accommodation statement describing what is needed must be attached.
  • Not medically fit. The DOB will not approve the application or renewal on that certification.

If the middle box is checked and no accommodation statement is attached, the submission is incomplete.

What the Physician Must Fill In

  • The physician’s full name and office address
  • A certification statement confirming the physician examined you with knowledge of your licensed duties and finds you qualified under DOB regulations
  • One of the three fitness checkboxes
  • A statement that a complete examination record is on file at the office, with the office address and date of exam
  • The physician’s signature and professional stamp

The stamp field is the one most often left blank. Check the form before you leave the office.

Renewals

Because the exam itself is only valid for 90 days, those two windows overlap neatly: schedule the physical when you start the renewal paperwork and both clocks will line up.

If your license lapses, you are into the reapplication process, which can carry requirements beyond a new physical. Do not wait until the last week before expiration to see a doctor.

Common Reasons the Form Gets Rejected

  • The exam was done more than 90 days before submission.
  • The license type marked on the LIC-62 does not match the license type on the DOB NOW application.
  • The physician signed but did not apply the professional office stamp.
  • The uploaded PDF is illegible where the signature or stamp appears.
  • “Medically fit with accommodations” is checked but no accommodation statement is attached.

Keep a copy of the submitted form and the physician’s contact information. The DOB may reach out to the doctor’s office directly, and the form itself certifies that the full examination record is kept there.
