Ohio OT license renewal happens every two years through the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. You renew online at eLicense Ohio, pay $70 plus a $3.50 processing fee, and complete 20 contact hours of continuing education before you submit.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal The deadline is June 30, and the board does not accept late renewals. Miss it and your license expires automatically, forcing you through reinstatement before you can practice again.
When Your License Expires
OTs and OTAs are on different cycles. OT licenses expire on June 30 of odd-numbered years. OTA licenses expire on June 30 of even-numbered years.2Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-1-05 – Biennial Renewal of License That puts the next OTA deadline at June 30, 2026, and the next OT deadline at June 30, 2027.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal
The renewal application opens April 1 of the expiration year, so you have roughly three months to finish. The board emails a renewal notice to the address on file. If you never see it, you are still responsible for renewing on time, which is why keeping your contact information current in eLicense matters.
Continuing Education You Need Before You Apply
You must complete 20 contact hours of CE during the two-year cycle ending June 30 of your expiration year. Hours do not carry over from a previous cycle.3Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-3-01 – Continuing Education
Two of those hours have specific subject requirements:
- At least one hour in ethics, jurisprudence, or cultural competence.3Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-3-01 – Continuing Education
- At least one additional hour in mental health or substance use. Presentations from the board’s Safe Haven program count for this requirement.3Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-3-01 – Continuing Education
The remaining 18 hours can cover any topic that directly enhances your clinical or professional skills as an occupational therapy practitioner. The mental health or substance use hour is easy to overlook, and missing it will hold up your renewal just as surely as being short on total hours.
How to Renew Online
All renewals go through eLicense Ohio at elicense.ohio.gov.4eLicense Ohio Professional Licensure System. eLicense Ohio Professional Licensure System Before you log in, have these ready:
- Your CE records: course titles, completion dates, and contact hours for each course from the cycle.
- Your current mailing address and email.
- Your workplace name and role.
- A credit or debit card for the $70 fee and $3.50 processing fee.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal
You do not upload CE certificates with the application. The board tells you to keep them in case of audit.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal What you type into the application becomes part of your licensing record, so check course titles and hour totals before you submit. A mismatch between your entries and your certificates can trigger an inquiry.
One boundary worth knowing: a criminal background check is not part of renewal. That requirement applies only to initial licensure.
If You Miss the Deadline
Ohio does not offer a grace period or a late renewal option. If you have not submitted a complete application, paid the fee, and met the CE requirement by June 30, your license expires automatically.2Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-1-05 – Biennial Renewal of License The board states plainly that you cannot renew late.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal
Once the license expires, you cannot practice occupational therapy in Ohio. Doing so is grounds for discipline under Ohio Revised Code 4755.11 and is a criminal offense under 4755.99, starting as a minor misdemeanor and escalating with repeat violations.2Ohio Legislative Service Commission. Reinstatement After an Expired License
Once a license has expired, reinstatement is the only way back. You apply through eLicense Ohio and must meet each of the following before the board will approve you:5Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-1-07 – Application for Reinstatement
- Twenty contact hours of CE completed within the two years before your reinstatement application. If you apply before August 1 of the year your license expired, hours earned for the renewal you missed can count, but they cannot also count toward your next renewal cycle.
- Passing the Ohio occupational therapy jurisprudence examination, which the board offers online at no additional charge.
- Viewing the required Safe Haven program presentation.
- Paying the reinstatement fee with your application.
If you have been out of practice for more than five years, the board can add further requirements such as additional CE, mentorship, competency assessments, or retaking the NBCOT certification exam.5Ohio Legislative Service Commission. Ohio Administrative Code 4755:1-1-07 – Application for Reinstatement You cannot practice until the board approves the application. Any reinstatement application left incomplete for a full year is treated as abandoned, and fees already paid are non-refundable.
Escrow Status If You Are Stepping Away
If you plan to stop practicing for a while but want to avoid reinstatement later, you can place your license in escrow. This is Ohio’s inactive status, and it keeps the license from expiring while it is on hold.6Ohio Legislative Service Commission. Ohio Administrative Code 4755-3-05 – Escrow of License; Restoration
A license can stay in escrow for up to two consecutive renewal periods. By the third, you have to restore it to active status or let it expire. To restore from escrow, you need 20 hours of CE (including one hour of ethics), a passing score on the jurisprudence exam, and the restoration fee paid through eLicense.6Ohio Legislative Service Commission. Ohio Administrative Code 4755-3-05 – Escrow of License; Restoration
Timing matters when you restore. A restoration between January 1 and March 31 of an expiration year leaves your license expiring that same June 30. Restoring between April 1 and April 30 pushes the expiration to June 30 of the next expiration year. Miss April 30 and you must renew in escrow status or the license expires, which puts you back on the reinstatement track.
Audits and Keeping Your CE Records
The board runs a continuing education audit after each renewal cycle, selecting licensees at random after the renewal period ends.7Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT Continuing Education Audit Reference Guide If your name comes up, you will need to produce certificates of completion showing the course title, date, hours, and provider for each course you reported. Hold onto those records for at least two years after you renew.1Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. OT-OTA License Renewal Failing to produce documentation during an audit can lead to disciplinary action. Scanning each certificate the day you finish a course and saving it to one folder makes the audit letter a non-event instead of a scramble.
