To get your student cleared for Round Rock ISD athletics or marching band, you need to submit the current UIL Pre-participation Physical Evaluation through the district’s Rank One portal. Print both sides of the form, fill out the medical history at home, take it to a licensed provider for the exam and signature, then scan both sides as a single PDF and upload it to your student’s Rank One profile along with the district’s other required paperwork.1Round Rock ISD. Who Needs a Physical and How Often
Round Rock ISD requires an annual physical for every student-athlete and every marching band member, regardless of grade level.1Round Rock ISD. High School Athletics The district’s rule is stricter than the UIL minimum, which only requires a physical before junior high participation and again before the first and third years of high school.2University Interscholastic League. UIL Constitution and Contest Rules – Subchapter C: Athletics – Section 1205: Athletic Eligibility A current form has to be on file before your student can participate in any practice, scrimmage, game, or off-season athletic period. Marching band is covered by the same UIL form and rules as athletics.3University Interscholastic League. Athletic and Marching Band Pre-participation Physical Evaluation
Download the Current Year’s Form
Use only the current year’s UIL Pre-participation Physical Evaluation. The version approved for the 2026–2027 school year is on UIL’s athletics forms page and is also linked from Round Rock ISD’s Rank One portal. An older form will be rejected. A Spanish-language version is available on the same UIL page.4University Interscholastic League. Athletics Forms – Medical Forms
The form is two-sided. Side 1 is the medical history, which parents and students complete at home. Side 2 is the physical examination, which the provider fills in at the appointment.5University Interscholastic League. Preparticipation Physical Evaluation – 2026 Form Print both sides before you go in; the Rank One portal tells families to “print the forms, fill out the Medical History form, and take them to your doctor at the time of the physical exam.”6Round Rock ISD. Fill Out the Medical History at Home
The medical history side is on the parent and student. Finish it before the appointment, because most clinics won’t start the exam until the history is complete.
The form asks about previous concussions and how many, bone fractures, dislocated joints, sprains, and any lingering pain or swelling from old injuries. It also asks whether any family member died of heart problems or suddenly and unexpectedly before age 50.7Mabank Independent School District. Preparticipation Physical Evaluation – Medical History Pay close attention to the heart section, which asks about fainting, dizziness during exercise, chest pain, and racing heartbeat. Answer honestly even if episodes seemed minor.
List all current medications and any chronic conditions like asthma or diabetes. For every “yes” answer, write an explanation in the space provided or on an attached sheet. Both the student and a parent or legal guardian must sign and date the bottom of the medical history side.7Mabank Independent School District. Preparticipation Physical Evaluation – Medical History If either signature is missing, the clinic will send you home to redo it.
An electrocardiogram screening is mentioned on the form as optional. UIL does not require an ECG, but parents who want one can arrange it separately.2University Interscholastic League. UIL Constitution and Contest Rules – Subchapter C: Athletics – Section 1205: Athletic Eligibility
At the Doctor’s Appointment
The exam has to be done by one of these providers: a physician (MD), a doctor of osteopathy (DO), a physician assistant, an advanced practice nurse, or a doctor of chiropractic. A form signed by any other practitioner won’t be accepted.5University Interscholastic League. Preparticipation Physical Evaluation – 2026 Form
The provider records height, weight, blood pressure, pulse, and vision in both eyes, then works through a system-by-system checklist: heart auscultation lying down and standing, lymph nodes, and a screen for Marfan syndrome markers like pectus excavatum or joint hypermobility. For males, a genital exam is included if indicated. Each system is marked normal or abnormal with the provider’s initials.5University Interscholastic League. Preparticipation Physical Evaluation – 2026 Form
At the bottom of Side 2, the provider checks one of three clearance boxes: cleared for full participation, cleared after completing a specified evaluation or rehabilitation, or not cleared with the reason noted. The provider then prints their name, signs, and fills in the exam date, clinic address, and phone number.5University Interscholastic League. Preparticipation Physical Evaluation – 2026 Form
Before you walk out of the office, check that the clearance box is marked, the signature line is filled in, and the clinic contact information is written in. A form missing any of those fields will be sent back.
What to Expect to Pay
Most standard insurance plans do not classify a sports physical as a preventive service, so coverage varies by plan.8HealthCare.gov. Preventive Health Services At Texas urgent care and retail health clinics, a basic UIL physical generally runs around $40, climbing to $65 or more if the provider has extra paperwork or testing to do. Adding an optional EKG can push the total toward $100. Some pediatrician offices bundle the sports physical into an annual wellness visit, which insurance is more likely to cover. Watch for free or reduced-cost sports physical events hosted by local clinics and hospitals; Round Rock area providers frequently run these in June and July.
Upload the Form Through Rank One
Round Rock ISD uses Rank One as its electronic clearance system. The parent portal is at roundrockisd.rankone.com.6Round Rock ISD. Round Rock ISD – Rank One Portal
If you don’t already have an account, create one and link your student using their official RRISD student ID number and legal name. Nicknames will cause a mismatch. Select the correct campus and the correct school year during setup, because a profile under the wrong school won’t reach the right athletic trainer.
Once logged in, open the Electronic Participation Forms tab. You’ll see a list of required online forms that need to be signed digitally; complete and sign all of them. To upload the physical, scan or photograph both sides of the completed UIL form as a single PDF, then attach it to your student’s profile. Check that the image is legible and no edges are cut off before you submit. A confirmation message should appear after a successful upload. Keep your own digital or paper copy in case anything gets lost.6Round Rock ISD. Round Rock ISD – Rank One Portal
If you can’t upload digitally, you can hand a paper copy directly to the campus athletic trainer, who will enter it into the system.1Round Rock ISD. High School Athletics
Other Forms You Still Need
The physical by itself does not clear your student to play. Round Rock ISD also requires the following before any participation, including off-season workouts:1Round Rock ISD. High School Athletics
- The Emergency Medical Care Card, which has to be picked up in person from your campus athletic department. It is not available for download or online completion.
- The Round Rock ISD Extracurricular Code of Conduct, a signed acknowledgment of the district’s behavior expectations for extracurricular participants.
- The Round Rock ISD Athletic Code of Conduct, a separate acknowledgment specific to athletics covering training rules and disciplinary procedures.
Most of the acknowledgment forms can be signed electronically inside Rank One, but confirm with your campus athletic trainer which ones need a physical signature.
Confirming Your Student Is Cleared
After you upload everything, the campus athletic training staff reviews the medical data for anything missing or inconsistent. Processing slows down in late summer when hundreds of forms come in before fall sports. Check your student’s status by logging into Rank One and viewing their compliance indicator. If something is flagged as incomplete, the system notes what’s missing so you can fix it. Your student cannot practice or compete until the status shows full clearance.
