What the Form Asks For

What to Include in the Envelope
- A copy of your driver’s license showing your current name, address, and photo. If any of those changed since your original CWP was issued, the copy must reflect the update.

The Fee

If you’re replacing a lost card and updating your address in the same submission, you pay five dollars total. The statute does not stack the fees.
While You Wait, and What to Do With the Old Card
SLED verifies your information, prints a new card, and mails it to the residential address on the application. If you later find a card you had reported lost, send that one back too.
If You Also Moved
An address change carries its own legal deadline separate from the replacement process. You must notify SLED in writing within ten days of any change to your permanent address.
