Help Staying in Your Own Home

SDS coordinates several programs built around keeping people out of institutional care when possible.

Oregon Project Independence

Oregon Project Independence (OPI) is state-funded in-home help for people aged 60 and older who are not on Medicaid. People under 60 with a qualifying Alzheimer’s or dementia diagnosis may also qualify.4Oregon Cascades West Council of Governments. Oregon Project Independence Brochure Services include personal care, housekeeping, respite for caregivers, case management, home-delivered meals, adult day services, grocery shopping, and transportation help. Fees slide with income after medical deductions. Eligibility is set through an in-person assessment; call 541-967-8630 or 800-638-0510 to arrange one.

Medicaid Long-Term Services and Supports

If you qualify for Medicaid, SDS case managers coordinate Long-Term Services and Supports, which can be delivered at home or in a care facility. The in-home version, OPI-M, covers personal care, housekeeping, meal preparation, home-delivered meals, chore services, adult day services, assistive technology, and home modifications.5Oregon Cascades West Council of Governments. Hiring Your Own Care Worker

People receiving in-home services through Medicaid or OPI can hire and supervise their own provider through the Consumer-Employed Provider program. The Employer Resource Connection, funded by the Oregon Home Care Commission, offers free training, template interview questions, sign-in sheets, performance evaluations, and safety checklists.6Oregon Department of Human Services. Facility-Based Care Options

When living at home is no longer workable, SDS coordinates placements in three types of settings:

  • Adult Foster Homes: licensed single-family homes caring for up to five residents. OCWCOG is the local licensing authority and monitors compliance.7Oregon Cascades West Council of Governments. Adult Foster Homes
  • Assisted Living and Residential Care Facilities: licensed settings for six or more people with access to registered nurses.
  • Skilled Nursing Facilities: 24-hour nursing care from state-certified nursing assistants and nurses.

Before a nursing facility admission, SDS conducts a pre-admission screening to confirm that level of care is actually needed, and works with residents who could move to a less restrictive setting.3Oregon Cascades West Council of Governments. 2021-2025 Area Plan Draft
