SNAP Verification Documents Required in Louisiana

The SNAP verification documents required in Louisiana fall into five groups: proof of identity for each household member, proof of Louisiana residency, Social Security numbers, income documentation from every source, and, optionally, records of expenses that reduce your countable income. Gathering all of these before you file the Application for Assistance (form OFS 4APP) is the single best way to avoid the delays that come from missing paperwork.¹ SNAP is now administered by the Louisiana Department of Health after the program transferred from the Department of Children and Family Services under Project One Door.²

Identity, Social Security Numbers, and Residency

Every person listed on the application needs proof of identity. A driver’s license, state ID card, or birth certificate works. So do baptismal or christening records and medical records that show a date of birth.³

Each household member must also have a Social Security number on file, or proof that an application for one has been submitted. Without it, that individual becomes ineligible, though the rest of the household may still qualify.⁴

For residency, bring a current lease, mortgage statement, or utility bill in your name. If you don’t have any of those, your landlord can complete form OFS 81 to verify where you live.³

Non-Citizen Household Members

Non-citizens can qualify for SNAP but need verifiable USCIS documentation showing eligible immigration status.⁵ If documentation has been submitted to USCIS and verification is still pending, the state cannot deny benefits solely because the response hasn’t returned.⁶

Income Documents

Income verification is mandatory for every household before certification, and your benefit amount is calculated directly from what you report.⁶

For wages, the state uses the last four consecutive pay periods to figure your average monthly earnings. Bring pay stubs covering that window. Your employer can also complete form OFS 87, a wage verification letter confirming gross income for the same period.⁷ Self-employed applicants should bring records of gross receipts and business expenses.

For unearned income (Social Security, SSI, SSDI, pensions, VA benefits, unemployment, child support), bring the official award letter, agency statement, or bank records showing the recurring deposits. A Social Security benefit verification letter can be requested through SSA’s website or by calling their office.⁸

Bank Statements and Resource Records

Not every household has to verify assets. Households categorically eligible through TANF can skip the resource test entirely.⁹ If yours does need to verify, current resource limits are $3,000 for most households and $4,500 for households with someone aged 60 or older or a person with a disability. These figures adjust annually.¹⁰

Bring recent bank statements showing the current balance in every checking and savings account. Your home and the first vehicle in the household don’t count toward the limit.

Expense Documents That Raise Your Benefit

Documenting expenses is technically optional. In practice, skipping it costs money every month, because verified expenses reduce your countable income and increase your benefit.

Shelter Costs

Bring the most recent rent receipts or mortgage statement, plus separate bills for property taxes and homeowner’s insurance if you pay those on your own. Recently moved? Include proof of the shelter costs at your current address.³

Utility Costs

Louisiana uses a Standard Utility Allowance that substitutes a fixed dollar amount for actual heating and cooling costs.¹¹ To qualify, show a recent electric, gas, or water bill, or proof that you receive energy assistance under LIHEAP. If your actual utility costs are higher than the SUA and you want to claim them, you have to verify every expense.

Court-Ordered Child Support

If someone in your household pays legally required child support to a person outside the home, the payment amount is deductible. Bring the court order establishing the obligation plus proof the payments are actually being made, such as pay stubs showing garnishment or receipts from the court clerk.

Medical Expenses for Elderly or Disabled Members

Households with a member age 60 or older, or with a documented disability, can deduct out-of-pocket medical expenses above $35 per month. That covers insurance premiums, prescriptions, copays, transportation to appointments, and similar healthcare costs. Bring pharmacy receipts, insurance statements, and doctor bills. This deduction is frequently missed and can meaningfully raise benefits for households with real medical spending.

Work Registration and Exemption Proof

Most able-bodied adults must register for work and accept suitable employment to keep SNAP.⁵ If a physical or mental health condition keeps you from working, you’re exempt, but the state needs documentation supporting that limitation, typically a statement from a treating provider.¹²

ABAWD rules are shifting under the One Big Beautiful Bill Act of 2025, and USDA is still developing guidance. Contact LDH directly for the current rules before you apply.¹²

Submitting Documents and What Happens If Any Are Missing

The fastest way to send verification is through the CAFE online portal, which accepts scanned uploads.¹³ You can also fax, mail, or drop off documents at your local parish office.¹⁴ Keep copies of everything. Documents get lost, and backups save you from starting over.

A caseworker will schedule a phone or in-person interview to review what you sent. The state has 30 days from your application date to issue a decision. Missing the interview or failing to provide requested verification results in denial. Even applicants who qualify for expedited processing still have to complete the full verification process afterward, or their case can be closed.¹⁵

Documents You’ll Need Later

Verification doesn’t end at approval. While receiving benefits, you have to report certain changes and attach proof, and you’ll recertify every 6 to 12 months depending on your household type.

Report the following with supporting documentation:

Address or housing cost changes, including new rent, mortgage, or utility amounts.
Anyone who moves into or out of the household.
Earned income changes of more than $100 in gross monthly pay, anyone starting or stopping work, or weekly hours dropping below 20.
Unearned income changes of more than $100 per month, or anyone starting or stopping benefits from Social Security, unemployment, pensions, or child support.

At recertification, LDH sends a Simplified Report (form OFS 4SR) with a printed deadline. Miss the deadline and your case can be closed.¹⁴ The Simplified Report can go back through the CAFE portal, by fax, by mail, or in person, and you attach proof for any changes just as you did the first time around.
