The SR16 form in Indiana is a court-generated document, officially titled the Certification of Indiana Abstract of Court Record, that Indiana courts use to tell the Bureau of Motor Vehicles about traffic convictions, missed court dates, and unpaid citations. You don’t fill it out. The court sends it to the BMV, and once it arrives, points can land on your driving record and your license can be suspended.
What an SR16 Reports
Under Indiana administrative rules, an “abstract of court record” is the SR16 or another BMV-approved form courts use to report violations, and “violation” is broad: felony and misdemeanor convictions, infraction adjudications, juvenile delinquency adjudications, and local ordinance violations all qualify.1Cornell Law School Legal Information Institute. Indiana Administrative Code 140 IAC 1-1-1.5 – Definitions
An SR16 tells the BMV one of four things about a driver:
- Convicted of a motor vehicle traffic offense
- Granted Specialized Driving Privileges on the underlying conviction
- Failed to appear for a scheduled court date on a traffic citation
- Failed to pay a citation for a traffic law violation
The same form is used to notify the BMV when any of those orders are later rescinded or voided so the record can be updated.2IN.gov. What Are Some Common Suspension and Reinstatement Forms
Courts don’t sit on these. Indiana law requires them to report traffic convictions (other than non-moving violations) to the BMV within 10 days of the conviction date.3Indiana Courts. How It Affects Your License
Once the BMV receives an SR16, two things can follow. The conviction is added to your driving record with any associated points, and the BMV may impose or record a suspension.3Indiana Courts. Processing Motor Vehicle Violations SR16 / BMV Court Record Abstracts
Points on Your Record
Points stay active on your driving record for two years from the conviction date. Common values:4IN.gov. Indiana Driver’s Manual – Chapter 5
- 2 points: speeding 1–15 mph over the limit, failure to use headlights
- 4 points: speeding 16–25 mph over, disregarding a stop or yield sign, using a hand-held device while driving
- 6 points: speeding 26+ mph over, failure to yield to an emergency vehicle, following too closely
- 8 points: driving while suspended, speed contest on a public road
Accumulating enough points can bring additional administrative consequences from the BMV on top of anything the court imposed.
Suspensions for Failing to Appear or Pay
An SR16 reporting a failure to appear or failure to pay lets the BMV impose an administrative suspension. A failure-to-pay suspension lasts three years, starting 30 days after the BMV mails the suspension notice.3Indiana Courts. Checking Whether an SR16 Has Hit Your Record
The quickest check is the BMV’s online portal. Create a free myBMV account and open your Viewable Driver Record (VDR), which shows current and resolved suspensions, citations, violations, and other entries at no charge.6Bureau of Motor Vehicles. Driver Record
For a certified copy, or to see the exact reinstatement fee owed on each suspension, order an Official Driver Record (ODR) for $4 by submitting a completed Request for Certified Records form to the BMV. The ODR also carries a reinstatement fee access code you’ll need to pay online.7Bureau of Motor Vehicles. Reinstating Your Driving Privileges
Fixing the Suspension
Reinstatement runs on two tracks: the court and the BMV. The BMV can’t fix a court-side problem, and the court can’t fix a BMV-side problem, so both usually need attention.
On the court side, that means appearing for the missed date, paying the citation, or completing whatever else the court ordered. Once the court is satisfied, it sends an updated SR16 to the BMV showing the issue resolved.
On the BMV side, you’ll typically owe a reinstatement fee. The amount depends on the type and number of suspensions on your record and is listed in the Reinstatement Requirements section of your ODR.7Bureau of Motor Vehicles. Reinstating Your Driving Privileges
The SR22 Piece
Certain suspensions also require you to file an SR22, which is proof of future financial responsibility. Your insurance company files it electronically with the BMV, and you must maintain continuous SR22 coverage for 180 consecutive days to satisfy the requirement. An SR22 policy can’t be cancelled without the insurer notifying the BMV first, so any lapse is flagged right away.8Bureau of Motor Vehicles. Proof of Financial Responsibility
For failure-to-appear and failure-to-pay suspensions specifically, submitting an SR22 can stay the suspension even before you fully resolve the court matter.5Bureau of Motor Vehicles. Driving on a Suspended License: Specialized Driving Privileges
An SR16-triggered suspension doesn’t always mean you can’t drive at all. Indiana allows courts to stay a suspension and grant Specialized Driving Privileges, which permit restricted driving, typically for work, school, or medical appointments. You request them by filing a verified petition in the court that ordered the suspension, and you must serve the petition on both the BMV and the prosecuting attorney.9Indiana General Assembly. Indiana Code Title 9, Article 30, Chapter 16, Section 9-30-16-3 – Stay of Suspension; Specialized Driving Privileges
Not everyone is eligible. SDP is unavailable if you:
- Have never been an Indiana resident
- Were convicted of operating a motor vehicle in a way that caused someone’s death
- Were found incompetent or unfit to drive by the BMV
- Were suspended for illegally passing a school bus
- Have multiple prior convictions for violating SDP conditions connected to offenses involving serious bodily injury or death
If the court grants SDP, you must keep proof of financial responsibility insurance for the entire privilege period, carry a copy of the court order in any vehicle you drive, and produce it on request from a police officer. SDP does not authorize you to operate a vehicle that requires a commercial driver’s license.10IN.gov. Driving Privileges – Trial Court Information
Violating an SDP condition is serious. The court that granted the privilege can modify or revoke it, or order the BMV to lift the stay and impose the original suspension in full.10IN.gov. Driving Privileges – Trial Court Information
Effect on Out-of-State Licenses
An Indiana suspension follows you across state lines. Indiana has been a member of the Driver License Compact since 1967, an agreement built on the principle of “One Driver, One License, One Record” under which member states share information about suspensions and traffic violations by out-of-state drivers.11National Center for Interstate Compacts. Driver License Compact If you hold an Indiana license and get convicted in another compact state, Indiana can treat the offense as if it happened here. If you move away while carrying an Indiana suspension tied to an SR16, your new state will likely see it when you apply for a license. Parking tickets and other non-moving violations aren’t reported through the compact, but anything on an SR16 is.
SR16 vs. SR21
These get confused often. The SR16 is a court-to-BMV form about convictions, missed court dates, and unpaid citations. The SR21, the Indiana Operator’s Proof of Insurance Crash Report, is an accident form your insurance company sends to the BMV after a crash involving bodily injury, death, or significant property damage. Different form, different sender, different purpose. If a single incident produced both a crash and a citation, both forms can appear, but they travel separate paths.
