The Texas DL-64 change of address form is the one-page application you use to update the address on your Texas driver license or ID card, or to replace one that’s been lost, stolen, or damaged. It is not the form for a new license or a renewal — those use the DL-14A. If you’ve moved, Texas gives you 30 days to report it, and the DL-64 is how you do it by mail.1Texas Department of Public Safety. DL-64 Application for Change of Address or Replacement
The 30-Day Deadline
Texas Transportation Code Section 521.054 requires license and ID card holders to notify DPS within 30 days of moving and to apply for a duplicate card showing the new address. You’ll need your old address, new address, and license or ID number to complete the update.2State of Texas. Texas Transportation Code TRANSP 521.054
If you’re cited for missing the deadline, a court can dismiss the charge if you correct the address within 20 working days and pay a reimbursement fee of up to $20. The court can waive that fee in the interest of justice, but the safer path is filing on time.
Who Can Use the DL-64 by Mail
The mail option is limited. All of the following must be true:3Texas Department of Public Safety. How to Change Information on Your Driver License or ID Card
- Your license is current — not expired, suspended, or revoked.
- You hold a non-commercial license: Class C, CM, or a Class A or B that is not a CDL. CDL holders cannot mail the DL-64.
- You are at least 18. Provisional and learner license holders must go in person.
- Your Social Security number is already on file with DPS.
- You are a U.S. citizen.
If any one of these doesn’t apply to you, use the online system or visit a DPS office instead.
Filling Out the Form
Download the DL-64 from the DPS website or pick one up at a driver license office. Complete it in black ink and print legibly; smudged entries can delay processing.
The top section asks for your full legal name, date of birth, driver license or ID number, and Social Security number. Below that, enter your old address and your new address. Every field should match what DPS already has on record for your name and date of birth. A small discrepancy can flag the form for manual review.
The form contains a sworn statement certifying that the information is true. You’ll also indicate your dwelling type — single-family home, apartment, motel, or temporary shelter. Sign and date the form before mailing. Unlike the DL-14A, the DL-64 does not require a staff member to witness your signature.4City of Grandview. DL-64 Texas DPS Form

After You Submit
DPS prints and mails the replacement to your new address. Cards typically arrive within two to three weeks of the transaction being processed.8Texas Department of Public Safety. You can track production and shipping through the DPS tool at dps.texas.gov. If the card doesn’t arrive in the expected window, DPS customer service takes inquiries through the email form on that page.
Hang on to your old card until the replacement arrives. If the old card was lost or stolen, you’ll have no physical card during the wait. Carrying a copy of the completed DL-64 and your mailing receipt can help explain the gap to law enforcement, though it is not an official substitute for a license.
