Where to Apply

The permit office is not at Town Hall. It sits under the Parks, Recreation and Cultural Affairs department at 50 Irish Lane, East Islip, NY 11730. Phone is (631) 224-5335, and the TTY line is (631) 224-5397.1Town of Islip. Who Qualifies

Two things have to be true. You have to live in the Town of Islip, and you have to meet the state’s medical standard for a “severely disabled person” as set out in New York Vehicle and Traffic Law Section 404-a and detailed on the MV-664.1 application.2New York State Senate. Qualifying conditions include limited or no use of one or both legs, inability to walk 200 feet without resting, severe neuromuscular or orthopedic problems, Class III or IV cardiac conditions, certain lung disease measurements, legal blindness, use of portable oxygen, or another physical or mental condition that imposes unusual hardship in getting around.4New York State Department of Motor Vehicles. Permits come in two versions. A permanent permit covers long-term qualifying conditions. A temporary permit is for a short-term condition that leaves you unable to walk without a cane, crutches, walker, or similar device.5New York State Department of Motor Vehicles. The Application Form and Medical Certification

The form is New York State MV-664.1. You can download it from the NY DMV website or pick one up from the Islip office.1Town of Islip. Disabled Parking Permits You complete the top with your personal information. Your healthcare provider completes Part 2, describing the condition and confirming it meets the state’s standard. For a temporary permit, the provider also notes an expected recovery date.4New York State Department of Motor Vehicles. Who can sign depends on the type of permit:

  • Permanent permit: a Medical Doctor, Doctor of Osteopathy, Physician Assistant, Nurse Practitioner, Doctor of Podiatric Medicine (foot-related disabilities), or Optometrist (blindness).
  • Temporary permit: only a Medical Doctor or Doctor of Osteopathy.

If your provider prefers, they can skip Part 2 and instead write a statement on office letterhead describing the disabling condition and confirming it qualifies under the law. That letter can be submitted in place of the medical portion of the form.6New York State Department of Motor Vehicles. Parking for People with Disabilities

How to Submit and What to Bring

Bring the completed application and your New York State Driver’s License or Non-Driver ID. The issuing agent has to write the last three digits of that ID number on the face of the permit, so the card has to be shown at issuance, and it also proves you live in Islip.2New York State Senate. You can apply in person at 50 Irish Lane, and the permit is issued on the spot. You can also mail the application with a clear photocopy of your ID to the same address, and the office will mail the permit back. There is no online application for Islip at this time.1Town of Islip. Disabled Parking Permits

How Long the Permit Lasts and How to Renew

Temporary permits are valid for up to six months.5New York State Department of Motor Vehicles. Parking for People with Disabilities – The Law Permanent permits last longer, for a period set by the state DMV commissioner. Both carry a printed expiration date, and an expired permit cannot legally be used.

Contact the Islip office before your permit expires. A temporary renewal always needs fresh medical certification confirming the condition still exists. Permanent renewals are generally simpler because the condition is not expected to improve. Either way, you will present your driver’s license or Non-Driver ID again so the agent can verify residency and mark the new permit.2New York State Senate. Using the Permit Correctly

Hang the permit from your rearview mirror only when parked, and take it down before you drive. The NY DMV is direct about this: leaving it up while driving obstructs your view and is grounds for a stop.5New York State Department of Motor Vehicles. Parking for People with Disabilities – The Law

The permit belongs to you, not to a vehicle. You can use it in any car, truck, or van as long as you are the driver or a passenger. The vehicle cannot park in an accessible space using your permit if you are not being transported in it. Carry your driver’s license or Non-Driver ID whenever you use the permit; an officer can ask to see it, and the last three digits on the permit should match your card.2New York State Senate. New York Vehicle and Traffic Law 1203-A – Parking Permits for Handicapped Persons

One boundary worth flagging. The permit itself does not exempt you from paying at meters. New York offers a separate metered parking waiver for people whose disability physically prevents them from reaching or operating a meter, valid in cities, towns, and villages across the state other than New York City. You would apply for that waiver through your local issuing agent as a separate benefit.

Lost, Stolen, or Misused Permits

If your permit is lost, stolen, or damaged, the Islip office can issue a replacement. Call (631) 224-5335 for current instructions on what to bring.1Town of Islip. Disabled Parking Permits

The permit is not transferable. If someone else uses it while you are not in the vehicle, or if the privileges are otherwise abused, the issuing authority can revoke it. Knowingly making a false statement to obtain a permit carries a civil penalty of $250 to $1,000, in addition to any other legal consequences.2New York State Senate.
