The U8 modifier is a Healthcare Common Procedure Coding System code officially described as “Medicaid level of care 8, as defined by each state.”1AAPC. HCPCS Modifier U8 That national definition is deliberately blank. Each state Medicaid program fills it with its own meaning, and those meanings vary enough that a provider in Ohio and a provider in California using U8 on the same day are almost certainly telling the payer two completely different things. What U8 means on your claim depends on your state and on the procedure code it rides with.
Why “Level of Care” Does Not Mean What It Sounds Like
Read literally, the HCPCS language suggests a clinical acuity tier. In practice, U8 and its siblings (U1 through U9, plus UA through UD) work as blank slots that states fill with whatever administrative label their Medicaid programs need.2California Department of Health Care Services. Medi-Cal Modifier Appendix The same two characters can designate a provider credential in one state, a service setting in another, and a clinical finding in a third. There is no January 2026 CMS update changing these modifiers at the federal level.3Noridian Medicare. Modifier and HCPCS Changes for January 2026 To know what U8 means for a given claim, you have to look at the state’s Medicaid manual and the specific rule tied to the procedure code.
California: Justice-Involved Services
Medi-Cal requires U8 on all claims for justice-involved services, meaning health care delivered to incarcerated individuals up to 90 days before their release from correctional facilities under a federal 1115 demonstration waiver.4California Department of Health Care Services. Medi-Cal Justice-Involved Manual The modifier tags each claim so the state can identify and process it separately from standard Medi-Cal billing. California also uses U8 with HCPCS code J3490 to identify medroxyprogesterone acetate for contraceptive use, a family planning application that has nothing to do with the justice-involved program.2California Department of Health Care Services. Ohio: Three Different Meanings in One State
Ohio shows how elastic U8 can be inside a single Medicaid program. The Ohio Department of Medicaid assigns the modifier three separate definitions depending on the service category.5Ohio Department of Medicaid. Ohio Medicaid Modifiers
- In community mental health and substance use disorder billing, U8 designates a social worker assistant operating under general supervision, meaning a supervising practitioner is available by phone. It appears on services such as Community Psychiatric Supportive Treatment (H0036) and Psychosocial Rehabilitation (H2017).6CareSource. CMHC Behavioral Health Billing Guide
- For Federally Qualified Health Centers, U8 paired with procedure code T1015 identifies a chiropractic services visit; the FQHC bills T1015 with U8 on the first claim line and the specific procedures on subsequent lines.7Ohio Department of Medicaid. FQHC/RHC Basic Billing
- In the Home Care Waiver Program, U8 on billing code S5125 designates a Home Care Attendant Services visit provided in lieu of intermittent nursing.8Ohio Legislative Service Commission. Rule 5160-46-06.1
Using U8 with the wrong procedure code or in the wrong service context produces a line-item denial, so Ohio providers have to match the modifier strictly to the applicable rule.
Pennsylvania: Needs Group 4 Under the Office of Developmental Programs
Pennsylvania’s Office of Developmental Programs uses U8 as a Support Intensity Scale needs group modifier corresponding to Needs Group 4.9Pennsylvania Department of Human Services. ODP Bulletin 00-18-05 Individuals assessed at this level generally have the highest support requirements and command higher statewide reimbursement rates than Needs Groups 1 through 3.10Pennsylvania Department of Human Services. ODP Residential Fee Schedule The modifier appears across residential service categories including licensed residential habilitation, life sharing, supported living, medically complex life sharing, and 24-hour respite in both licensed group homes and respite-only homes.
Kansas: Health Homes Program
Kansas uses U8 inside its Health Homes program to identify two core services: Patient and Family Support (procedure code S0280) and Referral to Community and Social Support Services (S0281).11Kansas Medical Assistance Program. Health Homes Bulletin Adding the HE modifier alongside U8 identifies the service as part of the Severe Mental Illness Health Home type; omitting HE defaults the same code combination to the Chronic Condition Health Home type.
Massachusetts: Behavioral Health Screening Result
MassHealth gives U8 a narrow role tied to developmental screening. Billed with CPT code 96110, U8 indicates that a physician assistant employed by a physician or community health center administered a behavioral health screen and identified a behavioral health need. The companion modifier U7 covers the same provider type when no need is identified. Claims submitted without the correct U modifier are denied. The clinical determination rests on four factors: the screening result, the patient’s or parent’s stated concern, the patient’s history, and direct observation.12MassHealth. All Provider Bulletin 211
Minnesota: Children’s Clinical Care Consultation
Minnesota Health Care Programs use U8 with procedure code 90899 to designate a face-to-face children’s clinical care consultation lasting 5 to 10 minutes.13Minnesota Department of Human Services. Children’s Mental Health Clinical Care Consultation The upper limit of the range counts toward an annual benefit cap of 15 hours. Providers must pool all consultation minutes for a given child on a single day into one claim line, while keeping separate documentation for each session.
Nevada: Previously Required, Now Retired
Nevada Medicaid previously required U8 on initial claims for Provider Type 86, Specialized Foster Care. That requirement was removed for all claims with dates of service on or after September 18, 2023.14Nevada Medicaid. Web Announcement — Specialized Foster Care States periodically revise or retire their use of U-series modifiers, so a definition that was correct one year may not be the current rule.
How U8 Fits in the Wider U-Series
U8 sits inside a family of state-use modifiers running from U1 through U9 and continuing through UA, UB, UC, and UD. Each is formally described as a Medicaid level of care modifier, and each actually means whatever the state and the paired service code say it means. In California, U7 denotes services rendered by a physician assistant and U9 designates a licensed midwife; in Ohio, U7 means counselor trainee in the behavioral health context and FQHC vision services in the FQHC context.2California Department of Health Care Services.
