Washington State’s PFML Medical Certification Form: Filling, Filing, and Denials

The Washington PFML medical certification form is the document your healthcare provider signs to confirm you or a family member has a serious health condition that qualifies for Paid Family and Medical Leave. You fill out section one with your personal details and consent to release medical information; your provider fills out section two with the diagnosis, dates, and expected duration of care; and you submit the completed form with your application through the online portal or by fax to 833-535-2273.

Getting this form right is what separates a claim that pays out in three to four weeks from one that stalls in factfinding. Vague dates, illegible handwriting, and mismatched personal details are the errors that cost people the most time.

Which Certification Form to Use

Washington’s Paid Family and Medical Leave program offers three versions of the certification form, and you need the one that matches your reason for leave:

  • Medical Leave Certification, for your own serious health condition.
  • Family Leave Certification, for caring for a family member with a serious health condition.
  • Pregnancy and Birth Certification, for pregnancy-related conditions and postnatal recovery.

All three are downloadable from the healthcare providers page at paidleave.wa.gov.1Washington State’s Paid Family and Medical Leave. Health Care Providers The state will also accept a federal FMLA certification form or a doctor’s note in place of its own form, as long as the substitute contains all the same information the PFML form asks for.2Washington State’s Paid Family and Medical Leave. The two programs cannot share medical documentation between themselves, so you have to submit it to the state yourself.3University of Washington. PFML Eligibility and Benefits

Filling Out Your Section

Section one is short but worth slowing down for. You provide your personal details and sign the consent that authorizes your healthcare provider to release medical information to the state.

Check three things before handing the form to your provider: your legal name spelled exactly as it appears on your PFML application, your date of birth, and your contact phone and email. Any mismatch between the name or birth date on your certification form and the name or birth date on your application will pull your claim into factfinding, where a specialist has to call you to reconcile the discrepancy. That adds weeks.

What Your Provider Must Certify

Your healthcare provider completes section two and signs it. The provider is certifying, on the record, that a serious health condition exists and that care or recovery requires time off work. Three pieces of information carry the most weight:

  • The date the condition began.
  • An estimate of how long you will need care or recovery.
  • Whether you need continuous leave or intermittent leave spread over time.

Dates must be specific. Answers like “unknown” or “indeterminate” for start dates or duration are not accepted, and a form with those entries will delay your eligibility determination.4Washington State’s Paid Family and Medical Leave. Pregnancy and Birth Certifications Paid Leave If your provider genuinely cannot pin down a return date, they should give a good-faith estimate with a specific date, not a blank or a hedge.

Providers are expected to return the completed form to you within seven calendar days of receiving it.1Washington State’s Paid Family and Medical Leave. Health Care Providers If you’re on a tight timeline (a scheduled surgery, a known due date), hand the form over early and remind the office of the seven-day expectation.

Corrections on the Form

If something on the form is wrong and needs to be fixed, the provider crosses out the incorrect information, writes the correction next to it, and initials the change. White-out is not accepted.1Washington State’s Paid Family and Medical Leave. Health Care Providers A form with white-out corrections is treated as altered and can be rejected.

Who Counts as a Qualifying Healthcare Provider

Washington defines “healthcare provider” broadly. Any of the following can complete and sign your certification form:

  • Physicians and osteopathic physicians
  • Nurse practitioners
  • Physician assistants
  • Nurse-midwives and midwives
  • Clinical social workers and clinical psychologists
  • Optometrists, podiatrists, and dentists
  • Physical therapists
  • Naturopathic physicians
  • Chiropractors, under limited circumstances

The full statutory list appears in RCW 50A.05.010 and WAC 192-500-090.1Washington State’s Paid Family and Medical Leave. Health Care Providers A signature from someone outside this list will not certify the condition, no matter how detailed the note.

What Qualifies as a Serious Health Condition

Your provider is certifying a serious health condition, so it helps to know what the term covers before your appointment. A serious health condition generally means an illness, injury, or physical or mental condition that involves either inpatient care (an overnight hospital stay) or continuing treatment by a healthcare provider.5U.S. Department of Labor. Taking Leave from Work When You or Your Family Member Has a Serious Health Condition under the FMLA

Continuing treatment typically involves a period of incapacity lasting more than three consecutive full calendar days, plus at least one visit to a provider within seven days of the first day of incapacity and either a prescribed course of treatment or a second visit within 30 days. Chronic conditions, pregnancy, and conditions requiring multiple treatments also qualify. A common cold or the flu usually does not meet the threshold unless complications arise.

Submitting the Completed Form

You have two ways to get the finished form to the state.

The first is uploading it as part of your online application. After you log in through SecureAccess Washington and work through the application screens, you upload your certification form along with your identity documents.6Washington State’s Paid Family and Medical Leave. Apply Now Scan or photograph the form so every field is clearly legible. Illegible uploads are treated as incomplete and will trigger a request for a resubmission.2Washington State’s Paid Family and Medical Leave. Get Ready to Apply

The second is faxing the completed certification form separately to 833-535-2273.4Washington State’s Paid Family and Medical Leave. Apply within 30 days of your qualifying event.2Washington State’s Paid Family and Medical Leave. Get Ready to Apply Waiting for a certification form is not a reason to delay filing the application itself past that window; submit the application on time and get the form in as soon as your provider returns it.

What Happens After You Submit

The state’s current processing time is three to four weeks.7Paid Leave Washington. About the Program If anything on your certification form is unclear (incomplete, contradicts your application, or is missing required dates), a specialist will call you. Answer the phone and return voicemails quickly. A specialist who cannot reach you closes the factfinding review with the information they have, which often means a denial or a reduced approval.

You can track your claim by logging into your benefit account and looking at “Claim Status” on the homepage.8Washington State’s Paid Family and Medical Leave. After You Apply The written decision arrives by mail and also appears in your account under “Correspondence.”

If the Certification Leads to a Denial

If your claim is denied because of something on the certification form (the provider isn’t on the qualifying list, dates were missing, the condition wasn’t found to meet the serious health condition standard) you can request a review through your online account. Log in, open the Claim Reviews section, click “Request Review,” select your claim, explain why you disagree, and upload supporting documents, including a corrected or supplemental certification if you have one.8Washington State’s Paid Family and Medical Leave. After You Apply If you applied on paper and don’t have an online account, call 833-717-2273 Monday through Friday between 8:30 a.m. and 4:30 p.m. to request a review by phone.

Review statuses move from “Submitted” to “In Review” to “Review Complete,” and the state mails the outcome. Reviews are processed in the order received, so a clean, well-documented resubmission of the certification form (with the missing dates filled in or a qualifying provider’s signature added) gives your review the best chance.
