  • U.S. birth certificate issued by a state or U.S. territory (abbreviated or abstract certificates are not accepted)

  • Dissolution of marriage or domestic partnership document showing the new legal name
  • Court-ordered name change document

If your name has changed more than once, you need a separate certified document for each change, forming an unbroken chain from your identity document to your current name.
