What Does Florida Blue Insurance Cover? Benefits and Exclusions

A Florida Blue health plan covers the ten categories of essential health benefits required by the Affordable Care Act, a full slate of preventive screenings at no cost when you stay in network, prescription drugs organized by tier, mental health and maternity care, and a range of specialty services from bariatric surgery to applied behavior analysis when they meet medical necessity rules. What Florida Blue covers in any given situation, though, depends on the specific plan you bought, because deductibles, copays, visit limits, prior authorization rules, and even which drug categories are included shift from policy to policy.1Florida Blue. The Essentials

The Ten Essential Health Benefits Every Plan Includes

Every ACA-compliant Florida Blue plan, whether individual, family, or small group, must cover ten categories of care:1Florida Blue. The Essentials

  • Outpatient care received without hospital admission
  • Emergency services, including ambulance transport
  • Hospitalization, including surgery and inpatient stays
  • Maternity and newborn care
  • Mental health and substance use disorder services
  • Prescription drugs
  • Rehabilitative and habilitative services and devices
  • Laboratory services
  • Preventive and wellness services, including chronic disease management
  • Pediatric services, including children’s dental and vision2Florida Blue. Affordable Care Act

Breastfeeding support and birth control are also required.1Florida Blue. The Essentials Adult dental and adult vision are not on the list, which is why Florida Blue sells them as separate BlueDental and BlueVision products.3Florida Blue. Dental and Vision Health

Preventive Care at No Cost

Routine preventive care is covered with no copay or deductible when you use in-network providers. Your annual wellness checkup carries a $0 copay and generally includes routine screenings, covered vaccinations, and lab work.4Florida Blue. Yearly Wellness Checkup Specific no-charge services include blood pressure, diabetes, and cholesterol screenings; cancer screenings such as mammograms and colonoscopies; depression, obesity, tobacco, and alcohol-misuse screenings; and standard CDC-recommended immunizations like flu shots and shingles vaccines. Women’s preventive services cover well-woman visits, cervical cancer screenings (Pap and HPV testing), contraceptive methods and counseling, and prenatal care.5Florida Blue. Preventive Care Services

There’s an important wrinkle. If a screening turns up something that needs follow-up testing or treatment, that follow-up is diagnostic rather than preventive and can be subject to your normal copays and deductible.4Florida Blue. Yearly Wellness Checkup

Prescription Drug Coverage

Florida Blue manages prescriptions through formularies it calls medication guides, which sort covered drugs into tiers that determine your copay. Plans use anywhere from two to seven tiers; individual and family plans typically run four to seven.6Florida Blue. Medication Guide In a common four-tier structure, Tier 1 generics carry the lowest copay (around $10 in one example), Tier 2 covers preferred brand names at a moderate copay, Tier 3 covers non-preferred brands at a higher copay after a pharmacy deductible, and Tier 4 holds specialty drugs at the highest cost.7Florida Blue. Coverage Requirements and Limitations

Three utilization tools can slow down or redirect a prescription. Prior authorization requires your doctor to get the insurer’s approval before coverage kicks in. Step therapy requires you to try a lower-cost alternative first. Quantity limits cap how much can be dispensed in a period, based on FDA dosing guidelines.7Florida Blue. Coverage Requirements and Limitations

Some drug categories may be excluded outright from certain plans, including medications for weight loss, fertility, and sexual dysfunction.8MyHealthToolkit Florida. Prescription Drugs Newly marketed drugs are also excluded until the insurer’s Pharmacy and Therapeutics Committee reviews them.9Florida Blue. Drugs Not Covered On the better side, select brand-name insulins carry reduced copays of $0 to $5 for a 30-day supply, and eligible plans let you get 90-day supplies of maintenance medications through home delivery, usually at a lower cost than retail.7Florida Blue. Mental Health and Behavioral Health

Mental health and substance use disorder treatment is covered across inpatient, outpatient, and telehealth settings, with individual, family, and group therapy typically included. Most plans don’t require a referral for outpatient mental health, though HMO plans are more likely to. Under the federal Mental Health Parity and Addiction Equity Act, individual and small-group plans must cover behavioral health at parity with medical and surgical benefits, so cost-sharing, treatment limits, and utilization rules can’t be more restrictive than for physical health conditions.10Florida Blue. Behavioral Health

Florida Blue runs behavioral health through a partner called Lucet, which members use to find licensed therapists, psychologists, psychiatrists, and counselors and book in-person or virtual appointments through the member portal. Virtual mental health visits are covered on most plans, often at the same rate as in-person sessions.11Florida Blue. Virtual Health Couples therapy is generally not covered, because insurance requires a diagnosable mental health condition for an individual; exceptions may apply when therapy is medically necessary for someone with a documented diagnosis.10Florida Blue. Behavioral Health

Maternity and Newborn Care

Maternity coverage is required on every ACA-compliant plan and includes prenatal visits, routine exams, ultrasounds, screenings for conditions like gestational diabetes, and nutritional guidance.12Florida Blue FWH. Expecting a Baby Labor and delivery coverage includes hospital room and board, delivery services from doctors and midwives, epidurals and emergency C-sections, and initial newborn assessments and vaccinations. Postpartum care includes follow-up checkups, lactation support, a breast pump (one per delivery), and mental health services for postpartum depression or anxiety.13Florida Blue. Maternity Care

Many members pay a single global maternity fee covering routine prenatal care, delivery, and postpartum care from their OB-GYN, with lab work and diagnostic testing billed separately. You must add a newborn to the plan within 30 days of delivery. Doula services are not currently a covered benefit.13Florida Blue. Maternity Care

Telehealth and Virtual Visits

Virtual visits for primary care, specialty care, and mental health are covered through phone, chat, or video. Many plans offer virtual primary care for as low as a $0 copay, and plans with a Value Choice Provider such as Sanitas Medical Group give you unlimited virtual visits at no additional cost, with the exception of HSA and Simple Choice plans.11Florida Blue. Virtual Health14Florida Blue. myBlue 2026 Virtual care is meant for non-emergency issues like colds, sore throats, headaches, mild allergies, skin rashes, anxiety, and depression. Providers can diagnose, treat, refer you to in-person care, and send prescriptions to a pharmacy. BlueVirtualCare, available on many plans, offers around-the-clock access to board-certified professionals.

Emergency and Urgent Care

Emergency room visits are covered on every Florida Blue plan, and under federal law emergency care is paid at in-network rates even when the hospital is out of network. The No Surprises Act protects you from balance billing in emergencies.15Memorial Healthcare System. Florida Blue Coverage Information16Florida Blue. Summary of Benefits and Coverage17Florida Blue. Rehab, Home Health, and Skilled Nursing

Physical and occupational therapy are covered when used to improve or restore function tied to illness, injury, surgery, or chronic conditions. The services must be medically necessary and follow a written treatment plan with measurable goals. Maintenance therapy aimed only at preserving your current level of function is not covered.18Florida Blue. Physical and Occupational Therapy Medical Coverage Guideline

Visit limits vary. One plan caps outpatient therapies and spinal manipulations at 35 combined visits per benefit period, inpatient rehabilitation at 30 days, home health care at 20 visits, and skilled nursing at 60 days.19City of Fort Walton Beach. Florida Blue Plan Benefits A 2026 HMO plan allows 60 home health visits and 60 days of skilled nursing with no charge for in-network care. Hospice is covered, though prior authorization may be required.20Florida Blue. Summary of Benefits – myBlue 2322US

Bariatric Surgery

Bariatric surgery isn’t a universal benefit. Whether a plan covers it depends on the policy, and when it does, the surgery must meet medical necessity criteria. For adults, that generally means a BMI of 40 or higher, or 35 to 39.9 with at least one related comorbidity such as type 2 diabetes, hypertension, or obstructive sleep apnea that hasn’t responded to medical management. Documented attempts at non-surgical weight loss and a psychological evaluation are also required.21Florida Blue. Bariatric Surgery Medical Coverage Guideline

Covered procedures include gastric bypass, sleeve gastrectomy, adjustable gastric banding, and biliopancreatic diversion with or without duodenal switch. Endoscopic procedures, intragastric balloons, and mini-gastric bypass are considered experimental and not covered. One plan document sets a $40,000 lifetime maximum for gastric sleeve surgery.22Florida Blue. Benefit Maximums Revision surgery may be covered for documented complications but not for stomach stretching from overeating.21Florida Blue. Bariatric Surgery Medical Coverage Guideline

Applied Behavior Analysis for Autism

Applied behavior analysis for autism spectrum disorder is covered when medically necessary. Prior authorization is required for all ABA services. There are no fixed visit caps; authorization follows clinical need. Comprehensive ABA treatment generally runs 25 to 40 hours per week of direct services, and focused treatment runs 10 to 25 hours. Treatment plans are reviewed for medical necessity at least twice a year, and coverage may be reduced if the patient doesn’t show meaningful progress.23Florida Blue. Durable Medical Equipment, Prosthetics, and Reconstructive Surgery

Florida Blue plans cover medically necessary durable medical equipment, prosthetics, and orthotics, and prior authorization is required for all three categories.24Florida Blue. Durable Medical Equipment Coverage Several items are excluded under the prosthetics policy: dentures replacing teeth, hairpieces for male-pattern hair loss, hearing aids, implants for cosmetic purposes, and deluxe devices with features beyond basic function.25Florida Blue. Prosthetics Medical Coverage Guideline

Cosmetic surgery, meaning procedures primarily to improve appearance without serving a significant physiological function, is excluded. Reconstructive surgery is covered when medically necessary to restore function or correct impairments from accidental injury, congenital defects, disease, or other surgical procedures, such as breast reconstruction after a mastectomy. Procedures done solely for psychological improvement or aesthetic enhancement are not covered.26Florida Blue. Cosmetic and Reconstructive Surgery Medical Coverage Guideline

Dental and Vision

Pediatric dental and vision are included as an essential health benefit in ACA plans.1Florida Blue. The Essentials Adult dental and vision are not, so Florida Blue sells BlueDental and BlueVision as separate products. BlueDental plans cover at least two regular exams and cleanings per year at little or no cost with an in-network provider. BlueVision plans include an annual eye exam at a low copay, a retail allowance for frames, full coverage on standard eyeglass lenses, a contact lens allowance, and discounts of 20% to 35% on laser surgery through a credentialed provider network.27Florida Blue. Care Outside Florida

If you travel or spend time outside Florida, the BlueCard program connects you to participating Blue Cross Blue Shield providers nationwide, with nearly 90% of U.S. doctors and over 80% of U.S. hospitals participating. You get in-network benefits and are protected from balance billing when you use BlueCard providers.28Florida Blue. BlueCard29Florida Blue. National and International Comparison Chart International travel runs through Blue Cross Blue Shield Global Core. Inpatient care at Global Core hospitals generally doesn’t require upfront payment, but outpatient care and services at non-participating international facilities typically require you to pay first and file a claim for reimbursement. A 24-hour assistance line is available for emergencies abroad.

What’s Commonly Excluded

Services that require prior authorization but are obtained without it may not be covered at all, leaving you responsible for the whole bill. Florida Blue maintains a regularly updated list of services requiring preapproval.19City of Fort Walton Beach. Florida Blue Plan Benefits

How Much You Actually Pay Depends on Your Plan

Coverage is only half the picture. What you pay is driven by your plan’s deductible, copays, coinsurance, and out-of-pocket maximum, and these numbers swing hard across the Florida Blue lineup. The myBlue 2129 plan for 2026, as one example, has a $0 medical deductible, a $2,100 pharmacy deductible, and an annual out-of-pocket maximum of $10,150 per person ($20,300 per family). Primary care visits are $35, specialist visits are $75, and a hospital stay is $3,000 per day up to $6,000 per admission.17Florida Blue. Summary of Benefits – myBlue 2129 Many 2026 myBlue plans advertise low or no-cost deductibles for qualifying members, primary care through Value Choice Providers for as low as $0, and over 150 generic prescriptions available at no charge.14Florida Blue. myBlue 2026

Plan type also shapes your access. HMO plans use a defined local network, require a primary care physician who coordinates care, may require referrals to specialists, and generally don’t cover out-of-network care except in emergencies. PPO plans have larger networks, don’t require referrals, and cover out-of-network care at a higher cost. EPOs sit in between with a smaller network than a PPO and no referral requirement. POS plans require a primary care doctor to coordinate care and provide referrals.30Florida Blue. Types of Health Plans

Because no two Florida Blue policies share the same cost structure, visit limits, or excluded drug categories, the only reliable way to confirm what yours covers is to read your plan’s Summary of Benefits or call the member services number on your ID card before you receive care.
