What Does MassHealth Limited Cover? Emergencies and Exclusions

MassHealth Limited covers emergency medical services only. It is a restricted form of Massachusetts Medicaid for residents whose immigration status, under federal law, keeps them from qualifying for broader coverage, and it pays for care only when a condition is acute and serious enough that waiting could cause real harm. Routine visits, preventive care, and ongoing treatment for chronic illness are not covered on their own, though a companion program called the Health Safety Net can pick up much of what Limited leaves out.

What Counts as an Emergency Under MassHealth Limited

The standard is federal. Section 1903(v) of the Social Security Act allows federal Medicaid dollars for nonqualified immigrants only to treat an “emergency medical condition,” and Massachusetts regulation 130 CMR 450.105(F) tracks that same language.1Mass.gov. Office of Medicaid MassHealth Review of the MassHealth Limited Program Emergency Medical In practice, that means symptoms that are acute rather than chronic, and severe enough that going without immediate care could reasonably be expected to put the patient’s health in serious jeopardy or cause serious impairment to bodily functions or organs.2Mass.gov. Chart of MassHealth Covered Services

That definition does the gatekeeping. A service that would be covered under MassHealth Standard without a second thought — a specialist follow-up, a therapy session, a routine cleaning — is not covered under Limited unless it is tied to an actual emergency.

Services MassHealth Limited Pays For

Within the emergency boundary, MassHealth Limited pays for:

  • Inpatient hospital emergency services, including labor and delivery.
  • Outpatient hospital emergency services, meaning emergency room visits at any hospital.
  • Medical services from doctors and clinics outside a hospital when the care relates to an emergency condition.
  • Pharmacy services for prescriptions needed to treat an emergency medical condition.
  • Ambulance transportation for an emergency medical condition.
  • Emergency treatment of cancer.
  • Emergency dental services.

These categories come from Massachusetts’s own descriptions of the program.3Mass.gov. MassHealth Coverage Types for Individuals and Families Including People With Disabilities4MA Health Connector. Program Information5Mass.gov. MassHealth Dental Member Handbook

Dialysis is a notable addition. Massachusetts is one of the states that treats dialysis as emergency care under Emergency Medicaid, covering it in both hospital and outpatient dialysis clinic settings, because a missed session can quickly become life-threatening.6IPRO ESRD Network Program. What MassHealth Limited Will Not Pay For

Anything outside the emergency definition falls outside the benefit. That rules out routine doctor visits, preventive care, well-child screenings, non-emergency dental work, vision care, eyeglasses, mental health counseling, substance use disorder treatment, physical therapy, chiropractic care, long-term care, nursing facility stays, and non-emergency transportation.3Mass.gov. The gap between Limited and other MassHealth coverage is wide. MassHealth Standard provides primary care, behavioral health, dental, vision, hearing aids, long-term care, and non-emergency hospital services. MassHealth CarePlus offers a broad benefits package including nursing and home health services. MassHealth Limited covers none of that on its own.3Mass.gov. If You’re Pregnant

Pregnancy changes the picture. People who become pregnant may qualify for MassHealth Standard regardless of immigration status, which covers prenatal care, labor and delivery, and postpartum care through 12 months after the end of the pregnancy.7Mass.gov. Even under Limited alone, labor and delivery is explicitly listed as a covered inpatient hospital emergency service, and infants born to mothers eligible for MassHealth Limited are automatically enrolled in MassHealth Standard from the date of birth.3Mass.gov. Filling the Gaps With the Health Safety Net

Many people on MassHealth Limited also qualify for the Health Safety Net, a separate state program that reimburses Massachusetts community health centers and acute care hospitals for medically necessary services to uninsured and underinsured residents. When both programs apply, the range of care a person can actually get is much wider.10Mass.gov. MassHealth Limited Health Safety Net

Through the Health Safety Net, eligible residents can receive:

  • Primary care and specialist visits, including physical therapy and immunizations.
  • Behavioral health and substance use disorder services, both inpatient and outpatient.
  • Dental and vision services.
  • Prescriptions, when written by a Health Safety Net clinician and filled at a Health Safety Net pharmacy.
  • Lab work and imaging.
  • Inpatient and outpatient hospital services.

Eligibility runs on income. Residents at or below 150% of the federal poverty level may have services fully covered, and those between 151% and 300% may qualify after meeting a deductible.10Mass.gov. MassHealth Limited Health Safety Net

One catch matters. The Health Safety Net only pays for services delivered at Massachusetts community health centers and acute care hospitals. Doctors in private groups working inside a hospital may not be covered, and patients can be billed separately for lab tests or imaging run by private companies rather than the hospital itself. Ask the provider what the Health Safety Net will pay for before the appointment. The program does not issue a member card; patients get an eligibility letter with an ID number to show to providers. MassHealth Limited members do receive a physical member card.10Mass.gov. MassHealth Limited Health Safety Net

Coverage Changes Coming in October 2026

Starting October 1, 2026, federal law will narrow Medicaid eligibility for certain lawfully present immigrants nationwide, and MassHealth estimates about 2,500 Massachusetts members will be affected.11Mass.gov. Federal Changes Affecting MassHealth Members After that date, only legal permanent residents, Cuban and Haitian entrants, and individuals from Micronesia, the Marshall Islands, and Palau will remain eligible for federally funded comprehensive Medicaid.

Groups that currently have comprehensive coverage and may be moved to MassHealth Limited include refugees, asylees, people with humanitarian parole, survivors of abuse or trafficking, noncitizen veterans, individuals with deportation withheld, pre-1980 conditional entrants, and certain Iraqi, Afghan, Ukrainian, and Amerasian immigrants.12Mass.gov. MassHealth Information for Noncitizens Non-disabled adults between 21 and 64 in these groups will generally be enrolled in MassHealth Limited and the Health Safety Net. Those 65 and older or disabled will generally go to MassHealth Family Assistance, which is more comprehensive. Children under 21, pregnant individuals, and those within 12 months of the end of pregnancy are exempt. MassHealth has said the details remain subject to change as federal guidance develops.11Mass.gov. Federal Changes Affecting MassHealth Members
