Mental Health and Substance Use Treatment
Behavioral health coverage comes in three layers, and adults have access to all of them.
For mild to moderate conditions that don’t significantly disrupt daily life, Medi-Cal covers mental health assessments, individual and group therapy, and psychiatric medications. These are coordinated through your managed care plan or primary care provider.6DHCS. Medi-Cal Behavioral Health Guide
For severe conditions like schizophrenia or bipolar disorder that interfere with daily functioning, Specialty Mental Health Services are delivered through county Mental Health Plans. Covered services include individual, group, and family counseling, medication management, crisis intervention and stabilization, day rehabilitation programs, targeted case management, residential treatment, and psychiatric inpatient hospitalization. County plans must meet timeliness standards: urgent appointments within 48 hours when no prior authorization is needed, and non-urgent outpatient mental health appointments within 10 business days.7Disability Rights California. Vision
Adults with full-scope Medi-Cal get a routine eye exam and eyeglasses once every 24 months. More frequent exams are covered when medically necessary, such as for eye pain, blurred vision, or diabetes-related eye monitoring; diabetic dilated exams can be covered annually.9DHCS. Equipment, Prosthetics, and Hearing Aids
Medi-Cal covers durable medical equipment including wheelchairs, walkers, canes, scooters, oxygen equipment, speech-generating devices, hospital beds, and patient lifts when prescribed by a licensed practitioner and found to be medically necessary. The program covers the lowest-cost item that meets the medical need, and unlike Medicare, it does not restrict use to the home. Community use is included.11Disability Rights California. Durable Medical Equipment – Medi-Cal, Medicare, and Dual Eligible Individuals
Prosthetic devices such as artificial limbs and breast prostheses are covered, as are orthotic devices like braces and compression garments. Larger purchases, rentals, and repairs trigger prior authorization requirements based on cost thresholds.12National Health Law Program. Medi-Cal Services Guide – Chapter 10
Hearing aids are covered but subject to an annual cap of $1,510 per beneficiary, with exemptions for pregnant women, children under 21, and residents of certain facilities.13National Health Law Program. Medi-Cal Services Guide – Chapter 12
Therapy and Other Specialty Services
Physical therapy, occupational therapy, and speech therapy are all covered. Physical therapy requires a prescription from a physician, dentist, or podiatrist, and prior authorization for all services. Prescriptions are limited to a six-month duration and must specify the procedures, frequency, and goals. A general order for “physical therapy” is not enough.14Medi-Cal Provider Manual. Physical Therapy Services
Occupational therapy needs a similar prescription. Initial and six-month evaluations don’t need prior authorization, but all other OT services do. Speech therapy requires a written referral from a physician or dentist and covers evaluation, treatment planning, and family-member instruction.15National Health Law Program. Medi-Cal Services Guide – Chapter 11
One cap to know about: for adults, occupational therapy, speech therapy, acupuncture, audiology, podiatry, and chiropractic share a combined limit of two visits per calendar month. Additional visits can be authorized through a Treatment Authorization Request when medically necessary.13National Health Law Program. Medi-Cal Services Guide – Chapter 12
Within that two-visit cap, several specialty services are available:
- Acupuncture is covered for the prevention or alleviation of severe, persistent chronic pain from a recognized medical condition. No prescription or prior authorization is needed for the first two visits per month.
- Chiropractic coverage is generally limited to manual manipulation of the spine, and only in certain settings such as emergency care, pregnancy-related services, or at federally qualified health centers.
- Podiatry covers treatment of foot, ankle, and tendon disorders that significantly impair walking. Routine nail trimming is not covered. Prior authorization is required beyond an evaluative office visit.
- Audiology is covered with a written referral from a physician or dentist and includes hearing evaluation, identification, and counseling.13National Health Law Program. Medi-Cal Services Guide – Chapter 12

Covered services include intermittent skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, home health aide visits, and home infusion therapy.21Medi-Cal Provider Manual. Home Health Services
In-Home Supportive Services is a separate, non-medical layer of support for aged, blind, or disabled adults who need help with housekeeping, meal preparation, bathing, and paramedical tasks like injections or wound care. Recipients act as the employer of their own caregiver, who can be a family member. Eligibility requires a Medi-Cal determination and an assessment by a county social worker.22California Department of Social Services. In-Home Supportive Services
Long-term stays in skilled nursing facilities, intermediate care facilities, and subacute care settings are covered when medically necessary. There is no income limit for long-term care eligibility, but most of a resident’s income goes toward the cost of care. Residents may keep $35 a month as a personal needs allowance, plus amounts for out-of-pocket medical premiums.23CANHR. Overview of Medi-Cal for Long-Term Care24LA County DPSS. Long-Term Care Medi-Cal
For adults who want to stay out of or leave a nursing facility, California runs several home and community-based waiver programs. The Home and Community-Based Alternatives waiver covers skilled nursing, home health aides, and therapy services in a home setting. The Assisted Living Waiver provides similar supports in residential settings. The Program for All-Inclusive Care for the Elderly serves adults 55 and older with personal care, therapy, and medical services through adult day health centers. Transportation to Appointments
Rides to and from medical, dental, mental health, and substance use appointments are covered, as are trips to pick up prescriptions and medical supplies. Two types are available:26DHCS. Transportation Services
- Non-medical transportation by car, taxi, bus, or other public or private vehicle for members who have no other way to get to an appointment. No prescription needed.
- Non-emergency medical transportation by ambulance, wheelchair van, or litter van for members who cannot safely use regular transportation. This requires a prescription from a licensed provider.
If you’re in a managed care plan, arrange rides through your plan’s member services department. Fee-for-service members use DHCS’s online scheduling system or call the telephone service center at (800) 541-5555.26DHCS. Newer Benefits: Doulas, Community Health Workers, and CalAIM
Since January 2023, Medi-Cal has covered doula services, including emotional and physical support during pregnancy, labor, birth, and the postpartum period up to one year, as well as support following miscarriage or abortion.28DHCS. Doula Services as a Medi-Cal Benefit Since July 2022, community health worker services have been covered as a preventive benefit, addressing chronic disease management, behavioral health, perinatal care, and violence prevention.29DHCS. Community Health Workers
The state’s CalAIM initiative, launched in 2022, added two more benefit categories for members with complex needs. Enhanced Care Management provides intensive care coordination for populations including people experiencing homelessness, individuals transitioning from incarceration, and those at risk of nursing facility placement. Community Supports offer services like transitional rent assistance (up to six months), housing navigation, medically tailored meals, recuperative care, and day programs as alternatives to more costly institutional care.30DHCS. What You’ll Pay
Most adult members pay nothing out of pocket. Adults who qualify at or below 138 percent of the federal poverty level receive free Medi-Cal with no cost-sharing. Members with income above that threshold may qualify under a Share of Cost arrangement, which works like a monthly deductible: you must incur a certain amount in medical expenses before Medi-Cal coverage kicks in that month. The share of cost is calculated by subtracting a maintenance need amount ($600 for an individual) from countable income. If you have no medical expenses in a given month, no share of cost is owed. You can apply unpaid medical bills and out-of-pocket costs for health items toward the share of cost, even for items Medi-Cal does not cover, as long as you give your county worker documentation.31CANHR. Changes Coming to Adult Coverage
Several policy changes are scheduled to affect adult Medi-Cal in the coming years. As of January 2026, asset limits of $130,000 for individuals were reinstated for members whose eligibility is based on age (65 and older), disability, or long-term care. Starting July 2026, undocumented adults aged 19 to 59 lose non-emergency dental benefits. By January 2027, adults in the Affordable Care Act expansion group face eligibility redeterminations every six months instead of annually and must report at least 80 hours per month of work, volunteering, or school to maintain coverage. Co-pays of up to $35 for certain services are set to take effect in October 2028 for expansion adults earning above 100 percent of the federal poverty level, though primary care, mental health, and substance use disorder services would be exempt.32Disability Rights California.
