Medi-Cal pays for a broad range of medical care, but several categories are off the table: cosmetic surgery, GLP-1 drugs prescribed for weight loss, erectile dysfunction medications, dental implants, in vitro fertilization, massage therapy, gym memberships, routine care received outside California, room and board in assisted living, and any service from a provider who is not enrolled in Medi-Cal. Other services are technically covered but capped so tightly that beneficiaries often hit the limit mid-treatment. Here is what Medi-Cal does not cover, and where the quiet limits sit inside the services that are supposedly included.
The Medical Necessity Test That Drives Every Denial
Almost every exclusion traces back to a single standard. Under California regulation, a service is covered only when it is reasonable and necessary to protect life, prevent significant illness or disability, or alleviate severe pain.1Legal Information Institute. California Code of Regulations Title 22 Section 51303 Anything that falls outside that definition is unlikely to be approved, no matter how much a patient or provider wants it.
Cosmetic procedures are the clearest example. Elective rhinoplasty, breast augmentation, and similar surgeries intended to change appearance rather than restore function are not covered. Reconstructive surgery after an injury or to correct a congenital condition can qualify, but only if the provider documents a functional problem, not an aesthetic one.
Clinical trials sit in a middle zone. Medi-Cal pays for routine patient care costs during a qualifying trial, meaning the standard labs and imaging you would need anyway. It will not pay for the investigational drug or device being tested.2California Medi-Cal Program. Clinical Trials Policy
Prescriptions Medi-Cal Won’t Fill
Medi-Cal Rx runs the pharmacy benefit, and several drug categories are excluded regardless of a prescriber’s recommendation.
GLP-1 medications prescribed solely for weight loss are no longer covered as of January 1, 2026. That includes Wegovy, Zepbound, and Saxenda when weight management is the only indication. GLP-1 drugs remain covered for type 2 diabetes with prior authorization, and Wegovy is still available for certain cardiovascular conditions and noncirrhotic metabolic dysfunction-associated steatohepatitis. Zepbound may be approved for obstructive sleep apnea. Beneficiaries under 21 keep coverage for weight-loss indications through the federal EPSDT benefit.3Department of Health Care Services. Changes to GLP-1 Drug Coverage – Effective January 1, 2026
Erectile dysfunction medications have not been a Medi-Cal benefit since 2006, when federal financial participation for them ended.4Department of Health Care Services. Medi-Cal Rx Contract Drugs List Over-the-counter dietary supplements, vitamins used for general wellness rather than a diagnosed deficiency, and gym memberships are not benefits either.
Even drugs on the formulary sometimes require prior authorization. If a pharmacy tells you a medication is not covered, ask your prescriber whether a prior authorization request or a therapeutic alternative might work before paying out of pocket.
Dental Services Adults Pay For Themselves
Children under 21 get comprehensive dental coverage through the EPSDT benefit.5Medicaid.gov. Early and Periodic Screening, Diagnostic, and Treatment Adult coverage is narrower. California restored full optional adult dental benefits on January 1, 2018, which brought back lab-processed crowns, posterior root canal therapy, periodontal services, and partial dentures.6Department of Health Care Services. Restoration of Adult Dental Services
Dental implants are not on that list. If you lose a tooth, Medi-Cal will typically pay for a partial denture or a bridge, but an implant can cost several thousand dollars out of pocket.
Orthodontic coverage is restricted to cases scoring 26 or higher on the Handicapping Labio-Lingual Deviation index, or meeting one of six automatic qualifying conditions such as cleft palate, severe traumatic deviation, or an overjet greater than 9mm with incompetent lips.7Department of Health Care Services. Handicapping Labio-Lingual Deviation Index California Modification Score Sheet Mild cosmetic crookedness does not qualify.
A bigger change is coming. Starting July 1, 2026, Medi-Cal will stop covering dental services for some adult members except for emergencies.8Department of Health Care Services. Medi-Cal Dental Benefit Changes If you are an adult beneficiary with planned dental work, check with your Medi-Cal dental plan well before that date to see whether your eligibility category is affected.
Vision and Hearing Limits
Medi-Cal does cover vision care for adults, including eye exams, low-vision evaluations, and prescription lenses.9DHCS. Vision What it won’t pay for is designer frames or elective refractive surgery like LASIK. Managed care plans may also require you to use in-network eye care providers.
Hearing aids, screenings, and audiologist visits are covered, but adults face an annual benefit cap of $1,510 per person each fiscal year running July 1 through June 30. That is enough for two analog hearing aids but may not stretch to premium digital models. Replacement batteries are not covered for adults. Hearing aids lost, stolen, or damaged beyond your control can be replaced outside the cap if you document what happened.10DHCS. Hearing Aid Cap FAQ
Therapies With Tight Caps
Several therapies are listed as benefits but limited in ways that surprise people.
Chiropractic coverage is restricted to spinal manipulation. No other service a chiropractor orders is covered. Chiropractic visits also share a combined two-visit-per-month cap with acupuncture, audiology, occupational therapy, and speech therapy.11California Medi-Cal Program. Chiropractic Services Two chiropractic visits in a month blocks acupuncture that month unless the provider gets a Treatment Authorization Request approved for additional visits.
Acupuncture is covered only for severe, persistent chronic pain from a recognized medical condition, and is subject to the same combined monthly cap.12California Medi-Cal Program. Acupuncture Services Acupuncture for stress relief or general wellness is not reimbursed.
Massage therapy is not a Medi-Cal benefit at all. Neither are weight-loss programs or gym memberships. In vitro fertilization is excluded, and California’s new IVF insurance mandate does not apply to Medi-Cal, so beneficiaries needing fertility treatment will need other funding.
Mental Health Services That Look Excluded But Aren’t
Medi-Cal covers mental health care, but the system is split in a way that produces apparent denials. Mild-to-moderate mental health conditions go through your Medi-Cal managed care plan’s provider network. Specialty mental health services for serious conditions like schizophrenia, bipolar disorder, or severe PTSD are carved out to the county’s Mental Health Plan. Substance use disorder treatment, including outpatient heroin detoxification, is also handled by county programs rather than managed care plans.
If your plan tells you a mental health service is not available, ask specifically whether you should contact your county Mental Health Plan. The care is usually covered somewhere; it just comes from a different door.
Long-Term Care and Assisted Living
Medi-Cal pays for medical care in a skilled nursing facility but does not cover room and board in an assisted living facility. Housing is treated as a personal living expense, not a medical service. California’s Assisted Living Waiver covers care services like personal care and home health aide support for qualifying individuals, but participants still pay their own room and board.13DHCS. Assisted Living Waiver The waiver is also limited to 15 counties and requires full-scope Medi-Cal with zero share of cost, age 21 or older, and care needs equivalent to nursing-facility level.
Inside a nursing facility, Medi-Cal pays for a shared room. A private room is only covered when a physician documents a medical reason for it. Personal comfort items like private television service or specialized phone lines are billed separately and are your responsibility.
Care Outside California or Outside the Network
Medi-Cal generally does not cover routine care received outside California. If you travel out of state and see a doctor for a non-emergency, you will likely owe the full bill. Federal regulations define an emergency medical condition as one with symptoms severe enough that a reasonable person would expect the lack of immediate treatment to seriously jeopardize their health, cause serious impairment to bodily functions, or result in serious organ dysfunction.14eCFR. 42 CFR 438.114 – Emergency and Poststabilization Services
For beneficiaries temporarily absent from California, Medi-Cal covers only emergency services within the United States and emergency services requiring hospitalization in Canada or Mexico.15Department of Health Care Services. Residency for Out of State Students Managed care plans must approve any non-emergency out-of-state services in advance. Even during a covered emergency, individual providers involved in the care may not accept Medicaid, leaving you responsible for those charges.16Department of Health Care Services. Coverage for All
Enrollment matters inside California too. If you see a specialist who is not enrolled in Medi-Cal, the program will not reimburse the cost. Verify enrollment before every appointment, especially for specialists and for facilities like imaging centers that may operate independently from the referring physician’s office.
If You Think Something Should Be Covered
Two routes exist when a service you need is not routinely covered.
First, a provider can submit a Treatment Authorization Request asking Medi-Cal to approve a specific service as medically necessary, including procedures that are not normally benefits. The request has to include the diagnosis, a signed physician order, the specific condition driving the need, and the type and frequency of the proposed services.17CA.gov Medi-Cal Providers. TAR Overview Building that clinical case is the provider’s job, not yours.
Second, if a service is denied, you have 90 days from the Notice of Action to request a State Fair Hearing, and the deadline can extend if illness or disability kept you from filing on time.18DHCS. Medi-Cal Fair Hearing One detail is worth protecting: if you request the hearing before the effective date on the Notice of Action, your benefits continue unchanged during the review. This is called aid paid pending, and it means you keep receiving the disputed service while the appeal plays out.
You can submit a hearing request by mail to the California Department of Social Services State Hearings Division, by fax to (833) 281-0905, online, or by phone at (800) 743-8525. Keep copies of everything. If English is not your first language, note your language and dialect on the request so the state arranges an interpreter.
