Medi-Cal pays for a wide range of care, but it draws hard lines around what counts as medically necessary and puts dollar caps or visit limits on several benefits it does provide. Knowing what Medi-Cal doesn’t cover — and where “covered” still comes with strings — can keep you from walking into a bill you weren’t expecting.
The program’s governing standard is narrow: services must be “reasonable and necessary to protect life, to prevent significant illness or significant disability, or to alleviate severe pain.”1Cornell Law. Cal. Code Regs. Tit. 22, § 51303 Anything outside that standard is generally excluded, and even services inside it may be rationed by cap, visit count, or prior authorization.
Services Medi-Cal Will Not Pay For
Several categories are flat exclusions. These are not benefits with limits; they are benefits Medi-Cal does not offer at all for most adult members.
- Cosmetic surgery. Procedures done solely to alter or improve normal appearance are excluded. Reconstructive surgery to correct abnormalities from congenital defects, trauma, infection, tumors, or disease treatment — breast reconstruction after a mastectomy, for example — is covered.2Blue Shield of California. Reconstructive Services Policy
- Experimental and investigational treatments. Drugs, devices, and procedures still in testing and not proven safe and effective are excluded. A narrow exception exists when conventional therapy has failed and the requested treatment is the lowest-cost option with a reasonable expectation of significantly prolonging life.1Cornell Law. Cal. Code Regs. Tit. 22, § 513033San Francisco Health Plan. Experimental or Investigational Services Policy
- Infertility treatment and IVF. Medi-Cal does not cover fertility treatments. California’s 2024 IVF coverage law (SB 729) explicitly exempts Medi-Cal enrollees.4CalMatters. IVF Health Insurance Coverage Law
- Comfort and convenience items. Air conditioners, humidifiers, exercise equipment, stair lifts, ergonomic chairs, specialty mattresses and pillows — items that don’t directly treat a medical condition — are excluded.5Health Plan of San Mateo. Member Handbook – Exclusions
- Custodial care. Non-medical personal care at home or in a facility that doesn’t require skilled nursing is not a benefit.6Health Net. Principal Exclusions and Limitations
- Common household supplies. Items that are not primarily medical in purpose, or that are common household goods, are excluded by state regulation.1Cornell Law. Cal. Code Regs. Tit. 22, § 51303
- Home and vehicle modifications. Structural home changes and vehicle adaptations are generally excluded. Some modifications may be available through CalAIM’s Community Supports program for qualifying members.6Health Net. Principal Exclusions and Limitations
- Circumcision past 30 days. For members 31 days and older, circumcision is not covered unless medically necessary.6Health Net. Principal Exclusions and Limitations
Weight-Loss Medications
As of January 1, 2026, Medi-Cal no longer covers GLP-1 medications prescribed for weight loss. Wegovy, Zepbound, and Saxenda are off coverage for that use.7Medi-Cal Rx (DHCS). Important Update: GLP-1s for Weight Loss Not a Covered Benefit Ozempic, Mounjaro, and Trulicity stay on the formulary but are restricted to treating type 2 diabetes; any claim with a weight-loss diagnosis will be denied.8Medi-Cal Rx (DHCS). 30-Day Upcoming Changes to Medi-Cal Rx
Narrow exceptions remain. Wegovy may be approved case-by-case for non-cirrhotic metabolic dysfunction-associated steatohepatitis (MASH) or cardiovascular disease, and Zepbound may be considered for obstructive sleep apnea. Patients under 21 can still get GLP-1 therapy for weight management with an approved prior authorization, because federal rules require broader coverage for children and adolescents.9California Medical Association. GLP-1 Medications for Weight Loss Will No Longer Be Covered by Medi-Cal
Dental Limits for Adults
Adult dental is covered but capped. For adults ages 21 to 54, Medi-Cal pays up to $1,800 per year for covered services, and routine exams, cleanings, and fluoride treatments are each limited to once every 12 months.10Smile California. Covered Services for Adults
The $1,800 cap is “soft” — care above it can still be approved with documentation of medical necessity. Emergency dental services, dentures, dental implants with implant-retained prostheses, and maxillofacial surgery don’t count against the cap at all.11Disability Rights California. Dental Services Through Medi-Cal Implants themselves are not a routine benefit; they are approved only when dentures cannot be worn for anatomical reasons, and each case needs a Treatment Authorization Request.12Mountainside Dental. Does Medi-Cal Cover Dental for Adults in 2026
Braces are covered only for members under 21, and only for qualifying conditions like handicapping malocclusion or cleft palate. Adults get no orthodontic coverage.13Smile California. Covered Dental Services Chart Crowns, root canals, periodontics, and sedation generally require prior authorization.11Disability Rights California. Dental Services Through Medi-Cal
Effective July 1, 2026, non-emergency dental services are no longer available to adult members (19 and up) who do not meet satisfactory immigration status requirements. Emergency dental care for severe pain, infections, and extractions is still covered for those members.14Health Consumer Alliance. Medi-Cal Changes and What You Need to Know
Vision and Hearing Caps
Vision
Adults get one routine eye exam every 24 months and one pair of eyeglass frames every two years.15National Health Law Program. Medi-Cal Services Guide, Chapter 12 Contact lenses are covered only when eyeglasses are not viable because of clinical conditions like keratoconus or aphakia. Protective, cosmetic, or job-related eyewear, progressive lenses, multifocal contact lenses, and vision therapy or visual training are all excluded.6Health Net. Principal Exclusions and Limitations
Hearing
Adult hearing benefits are capped at $1,510 per person per fiscal year (July through June). That cap covers the hearing aids, molds, supplies, repairs, an initial set of batteries, and up to six fitting and adjustment visits.16UCSF Ears. Medicaid Coverage for Hearing Care Replacement batteries for adults are not covered after that initial set.17Health Net. Hearing Coverage for Medi-Cal The cap can be exceeded with prior authorization for medical necessity, and bone-anchored hearing devices sit outside the cap as prosthetics.
Therapy, Chiropractic, and Acupuncture Visit Limits
Medi-Cal covers acupuncture, chiropractic, speech therapy, occupational therapy, and audiology for adults, but all five share a single combined ceiling: two visits per calendar month, total.6Health Net. Principal Exclusions and Limitations A chiropractor visit plus a speech therapy visit in the same month uses up both. More sessions can be authorized if medically necessary. Members under 21, pregnant members, and residents of skilled nursing facilities are exempt from the limit.18L.A. Care Health Plan. Medi-Cal Benefits Guide Highlights
Chiropractic coverage is limited to manual spinal manipulation. X-rays ordered by a chiropractor, maintenance care, and any ancillary diagnostic or therapeutic services from a chiropractor are not covered.19Health Net. Chiropractic Coverage for Medi-Cal Acupuncture is covered only for severe, persistent chronic pain from a recognized medical condition.18L.A. Care Health Plan. Medi-Cal Benefits Guide Highlights
Care Outside California
Medi-Cal’s geographic reach is narrow. Emergency and urgent care are covered anywhere in the United States and its territories, but routine and preventive care received outside California is not covered. Internationally, Medi-Cal pays only for emergency care requiring hospitalization in Canada or Mexico. Emergency, urgent, or routine care in any other country is not covered.20L.A. Care Health Plan. College Students Who Move to a New County or Out of California
Pharmacy and Over-the-Counter Limits
Prescription drugs run through Medi-Cal Rx, and some over-the-counter products are covered when they appear on the Contract Drugs List. OTC claims are capped at $50 per claim.21Medi-Cal Rx (DHCS). Medi-Cal Rx Contract Drugs List
Starting January 1, 2026, multivitamin combination products are no longer covered for adults 21 and over. Single-ingredient vitamins and dry-eye treatments now require prior authorization. Adult antihistamine coverage is restricted to generics, and prenatal vitamins are limited to use during pregnancy or lactation for patients ages 10 to 60.22California Medical Association. When a Covered Service Still Gets Denied
A benefit being “covered” doesn’t guarantee payment. Many treatments, medications, and pieces of equipment need prior authorization, and the request has to clear Medi-Cal’s medical necessity standard: reasonable and necessary to protect life, prevent significant illness or disability, or alleviate severe pain, with no comparable, less costly alternative.23Blue Shield of California Promise. Prior Authorization Review Policy
Services that commonly require prior authorization include inpatient hospital care, out-of-network specialist referrals, MRIs and CT scans, custom durable medical equipment, non-emergency medical transportation, and certain mental health treatments.23Blue Shield of California Promise. Prior Authorization Review Policy For medications, requests for off-label use or dosages above FDA-approved limits must include peer-reviewed medical journal articles supporting safety and effectiveness.24Medi-Cal Rx (DHCS). Medi-Cal Rx Prior Authorization Request Form Incomplete requests can be returned or denied.
If a managed care plan denies a treatment as experimental or investigational, members can request an Independent Medical Review through the Department of Managed Health Care. Qualifying generally requires a life-threatening or seriously debilitating condition, with a physician’s certification that standard therapies have been ineffective or are not appropriate.25Blue Shield of California Promise. Evaluation and Review of Experimental and Investigational Therapies
Clinical trials follow a split rule: Medi-Cal does not pay for the experimental drug or device being tested, but it does cover routine care costs tied to a qualifying trial for a serious or life-threatening condition, including diagnosis, monitoring, and treatment of complications. Data collection and non-clinical expenses like travel and housing are not covered.26DHCS. Clinical Trial Coverage Policy
Things People Assume Are Excluded But Aren’t
A few benefits are easy to miss. Gender-affirming care is a covered benefit; the Department of Health Care Services has stated that all medically necessary gender-affirming care is covered for Medi-Cal members of all gender identities, and plans may not categorically exclude it.27DHCS. Gender-Affirming Care Coverage
Doula services have been covered since January 1, 2023, including emotional and physical support during pregnancy, labor, birth, and up to one year postpartum, and support related to miscarriage and abortion.28DHCS. Doula Services as a Medi-Cal Benefit Private-duty nursing is covered for members under 21 who need continuous skilled nursing beyond what a visiting nurse provides, with prior authorization and detailed documentation.29Partnership HealthPlan. Private Duty Nursing Policy
CalAIM Community Supports can cover housing transition services, recuperative care after hospitalization, and medically tailored meals for members with qualifying chronic conditions. Plans aren’t required to offer every Community Support, and eligibility must be documented.30DHCS. Community Supports Policy Guide
Why Many of These Limits Don’t Apply to Children
Almost every adult limit above has a carve-out for members under 21. Under the federal Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit, Medi-Cal must cover all medically necessary services for children identified through screenings, even services that would not be covered for adults. That means no annual cap on hearing aids, more frequent vision exams and eyeglasses as needed, orthodontics for qualifying conditions, private-duty nursing, GLP-1 medications for weight management, and dental care without the $1,800 annual limit.15National Health Law Program. Medi-Cal Services Guide, Chapter 1231DHCS (Medi-Cal Dental). Orthodontic Seminar Packet If you have a child on Medi-Cal, assume broader coverage than the adult rules suggest and ask the plan to review under EPSDT.
