Which Weight Loss Medications Does Louisiana Medicaid Cover?

Louisiana Medicaid covers only three weight loss medications, and none of them are approved for weight loss by itself. The list is orlistat (Xenical), semaglutide (Wegovy), and tirzepatide (Zepbound). Each requires your prescriber to document specific medical conditions before the state will pay. Popular options like phentermine, Contrave, and Qsymia are not on the Louisiana Medicaid Preferred Drug List and are not covered.1Louisiana Department of Health. If you have type 2 diabetes and want a GLP-1, a different pathway applies and the rules are easier to meet. More on that below.

Orlistat: The Easiest to Get

Orlistat blocks the absorption of dietary fat and is the most accessible of the three. It does not require a prior authorization form. Louisiana Medicaid uses point-of-sale edits instead, meaning the pharmacy’s computer checks the requirements automatically when your prescription is filled.2Louisiana Department of Health. Orlistat (Xenical) POS Edits

To qualify, you must meet all of these:

  • At least 12 years old on the fill date.
  • A current BMI of 27 or higher, written on the dated prescription by your prescriber.
  • A valid diagnosis code for a condition that warrants orlistat use, such as high blood pressure, high cholesterol, or diabetes, submitted on the pharmacy claim.
  • Quantity capped at 90 capsules or a 30-day supply, whichever is less.

If any check fails at the counter, the claim is rejected automatically and your doctor cannot override it. Confirm your BMI and diagnosis appear on the prescription before you go to the pharmacy.2Louisiana Department of Health. Orlistat (Xenical) POS Edits

Wegovy: Covered as a Heart Drug, Not a Weight Loss Drug

This is where most people get stuck. Wegovy is widely marketed for weight loss, but Louisiana Medicaid added it effective July 1, 2024 specifically as a cardiovascular risk reduction medication after the FDA expanded its indication to include reducing heart attacks and strokes in certain patients.3Louisiana Medicaid. Semaglutide (Wegovy) Approval Criteria

You must meet all of the following:

  • Age 45 or older on the date of the request.
  • Documented BMI of 27 kg/m² or greater, with the date and most recent calculation included.
  • Established cardiovascular disease: a prior heart attack, a prior stroke (ischemic or hemorrhagic), or peripheral arterial disease. Peripheral arterial disease must be supported by evidence such as an ankle-brachial index below 0.85 at rest, a history of peripheral arterial revascularization, or amputation due to atherosclerotic disease.
  • No type 1 or type 2 diabetes. If you have diabetes, a different pathway applies.
  • Use alongside optimized heart disease treatment, lifestyle counseling, a reduced-calorie diet, and increased physical activity.

The cardiovascular documentation is where requests fail. A general note that a patient “has heart disease” is not enough. Your doctor needs to name the qualifying event or condition and provide supporting evidence.3Louisiana Medicaid. Semaglutide (Wegovy) Approval Criteria

Zepbound: Newer, With a Similar Framework

Zepbound (tirzepatide) appears on the 2026 Preferred Drug List with requirements that mirror Wegovy’s structure: a patient treatment agreement, clinical authorization, quantity limits, and treatment duration limits.1Louisiana Department of Health. Louisiana Medicaid Preferred Drug List (PDL) Because this is a recent addition, ask your prescriber to confirm the current approval criteria before assuming you qualify. The specific thresholds may differ from Wegovy’s.

GLP-1 Coverage If You Have Type 2 Diabetes

Type 2 diabetes disqualifies you from Wegovy under the cardiovascular pathway, but it opens an easier door. Ozempic (semaglutide) and Mounjaro (tirzepatide) are both on Louisiana’s Preferred Drug List under the diabetes category, and federal law requires state Medicaid programs to cover GLP-1 drugs approved for diabetes.1Louisiana Department of Health. Louisiana Medicaid Preferred Drug List (PDL)

Clinical authorization requires:

  • A diagnosis of type 2 diabetes mellitus.
  • A hemoglobin A1C of 6.5% or higher within the past six months. If your A1C has dropped below 6.5% but you have a history of 6.5% or higher, you may still qualify if you also have established cardiovascular disease.
  • For a non-preferred GLP-1, documentation of treatment failure, intolerable side effects, or a contraindication with the preferred options first.

Approvals last 12 months. Renewals require your prescriber to confirm you are responding positively to treatment.4Louisiana Medicaid. Updated Criteria for GLP-1 Receptor Agonists If you lose weight on Ozempic or Mounjaro, that weight loss is a side benefit of a covered diabetes drug. You do not need to meet Wegovy’s cardiovascular criteria.

How Approval Works

Orlistat clears at the pharmacy counter. Wegovy and Zepbound need prior authorization. For those, your prescriber completes the Louisiana Uniform Prescription Drug Prior Authorization Form.5Legal Information Institute. Louisiana Admin Code tit. 46, XLV-8001 – Louisiana Uniform Prescription Drug Prior Authorization For Wegovy, a separate Treatment Agreement must also be submitted. You initial six statements confirming that you have received lifestyle counseling, understand how to take the medication, will follow a reduced-calorie diet, and will increase physical activity. You and your doctor both sign it.6Louisiana Department of Health. Semaglutide (Wegovy) Treatment Agreement

Decisions typically come back within 24 hours of receipt as an approval, denial, or void.7Louisiana Medicaid. Louisiana Medicaid Pharmacy Benefits Management Prior Authorization Program If you are enrolled in a managed care organization rather than fee-for-service Medicaid, your MCO must align its criteria with state pharmacy standards, but submission procedures vary. Call your plan’s member services to confirm.

Staying on Wegovy: Renewal Depends on Weight Loss

Initial approval does not guarantee continued coverage. To renew Wegovy you must show one of the following:

  • At least 5% loss from your baseline body weight, or maintenance of a previous 5% loss. Your doctor must document weight before starting Wegovy and current weight, with dates for both.
  • Less than 5% loss, with a clinical explanation from your prescriber for why the goal was not met.

The renewal period depends on your results. More than 5% loss earns an additional six months. Less than 5% with clinical justification earns three months. If that three-month extension ends and you still have not reached 5%, no further approval will be granted.8Louisiana Department of Health. Semaglutide (Wegovy) Approval Criteria – Continuation of Therapy Your prescriber must also confirm at each renewal that you are continuing cardiovascular care and lifestyle changes.

What You’ll Pay

Louisiana Medicaid prescription copays are small, scaling from no copay for drugs costing $10 or less up to $2 to $3 for drugs costing more than $50. Total copayments across all your prescriptions are capped at 5% of your family’s monthly income. Members under 21, pregnant women, and Native Americans pay no copays at all.9Louisiana Department of Health. Louisiana Medicaid Pharmacy Copayments

If You’re Denied

Start with the quickest option. The prescriber who submitted the request can ask for reconsideration within 30 days of the denial by sending additional documentation. If a required detail was missing or your doctor can supply stronger evidence of cardiovascular disease, this is often faster than a formal appeal.10Louisiana Department of Health. How to Appeal Medicaid

If you are in a managed care plan, your next step is an appeal with your health plan through member services. Appealing within 10 days of the denial keeps your current services active while the appeal is reviewed. A decision usually comes within 30 days. If the plan upholds the denial, or if you are in fee-for-service Medicaid, you can request a State Fair Hearing within 120 days of the denial notice.11Louisiana Department of Health.
