In Alabama, the people who can legally inject Botox for cosmetic purposes are licensed physicians (MDs and DOs), certified registered nurse practitioners and physician assistants working under a board-approved protocol with a collaborating or supervising physician, and licensed dentists operating within a defined zone of the lower face. The Alabama Board of Medical Examiners treats cosmetic botulinum toxin injection as the practice of medicine, and anyone outside those categories who performs an injection commits a Class C felony.1Alabama Board of Medical Examiners & Medical Licensure Commission. Physicians
MDs and DOs have the broadest authority. A fully licensed physician can evaluate the patient, choose the treatment, and perform the injection with no supervisory arrangement, no additional protocol approval, and no dosage cap tied to the APP rules.1Alabama Board of Medical Examiners & Medical Licensure Commission. Notice – Botox Administration/AL Board of Nursing Declaratory Ruling
The physician is also the only person in a practice who may purchase the product. The ABME’s approved protocol states that the supervising or collaborating physician is “the only practitioner to purchase the product,” so the prescription drug must flow through a physician’s license even in practices where APPs do most of the injecting.3Alabama Board of Medical Examiners. Nurse Practitioners and Physician Assistants
CRNPs and PAs can inject cosmetic Botox, but only after clearing several regulatory steps. A CRNP needs a collaborative practice agreement with a licensed physician; a PA needs a registration agreement with a supervising physician. In both cases the physician-APP team must submit a cosmetic botulinum toxin injection protocol to the Board of Medical Examiners for approval before any training begins. For CRNPs, the Board of Nursing must also provide written approval.3Alabama Board of Medical Examiners. Cosmetic Botox Protocol Template4Alabama Board of Medical Examiners. Training Before Independent Practice
The APP must complete at least 10 hours of didactic training covering injectable safety, facial anatomy (nerves and blood vessels in particular), and board rules. The APP then observes 10 injection procedures and performs 50 more under the collaborating or supervising physician’s direct supervision. The physician evaluates the APP’s competency before independent practice begins, and continues to review the APP’s work on a defined schedule afterward.3Alabama Board of Medical Examiners. On-Site Physician and Dosage Cap
Whenever a CRNP or PA performs the injection, the collaborating, supervising, or a pre-approved covering physician must be physically on-site at the facility. That is stricter than the usual collaboration rules for nurse practitioners, which allow routine practice without on-site supervision.3Alabama Board of Medical Examiners. The protocol also caps an APP at 64 botulinum toxin units (or Botox unit equivalent) per treatment session for the cosmetic temporary paralysis of facial muscles. A physician performing the injection personally is not subject to that cap.4Alabama Board of Medical Examiners. Cosmetic Botulinum Toxin Injection Protocol for Advanced Practice Providers
Dentists, Within a Limited Facial Zone
Under Rule 270-X-2-.25, adopted by the Alabama Board of Dental Examiners, a licensed dentist may administer botulinum toxin in and around the mouth, teeth, gums, jaws, and adjacent structures. The rule defines “adjacent structures” with precise anatomical boundaries that roughly cover the lower face from the nasolabial folds to the jaw line. Forehead lines and crow’s feet sit outside that zone and outside a dentist’s legal scope in Alabama.5Alabama Board of Dental Examiners. Rule 270-X-2-.25 Administration of Botulinum Toxin and Similar Treatments by Dentists
Several conditions attach. The dentist must personally perform the injection; delegation to a hygienist or assistant is not allowed. The procedure must be part of a comprehensive dental treatment plan rather than a standalone cosmetic appointment. The dentist must have completed a board-approved training course within the preceding 24 months covering head and neck anatomy, pharmacology, safety and risk management, and live hands-on technique. Injections must take place in a dental clinic, not a home, salon, or any other non-clinical setting.5Alabama Board of Dental Examiners. Rule 270-X-2-.25 Administration of Botulinum Toxin and Similar Treatments by Dentists
In practice, a dentist might use Botox to treat TMJ disorders, bruxism, or facial pain, and may address cosmetic concerns in the same anatomical area as part of that plan.
Why Registered Nurses Are Not on the List
This is the point of most confusion in Alabama, because two state boards disagree. The Alabama Board of Nursing issued a declaratory ruling to an individual RN stating the nurse could administer botulinum toxin for cosmetic purposes. The Board of Medical Examiners publicly rejected that position, stating it “has not authorized the performance by, or delegation of this skill to, any person other than a PA or CRNP acting pursuant to a collaboration/registration agreement and the approved protocol.”1Alabama Board of Medical Examiners & Medical Licensure Commission. Notice – Botox Administration/AL Board of Nursing Declaratory Ruling
The ABME made two points worth understanding. A Board of Nursing declaratory ruling binds only that board and the specific nurse who requested it; it does not protect an RN in a malpractice lawsuit or in a disciplinary proceeding before the ABME. And any physician who delegates Botox injections to an RN “will not be acting in accordance with the approved protocols and may be in breach of the standard of care,” which puts the physician’s license at risk alongside the nurse’s.1Alabama Board of Medical Examiners & Medical Licensure Commission. Everyone Else Is Committing a Felony
Aestheticians, cosmetologists, medical assistants, and anyone marketing themselves as an “injector” without one of the credentials above are flatly prohibited from administering Botox in Alabama. Performing the injection without a license is the unlicensed practice of medicine, a Class C felony under Alabama Code 34-24-51.2Alabama Legislature. Alabama Code 34-24-51 – Practicing Medicine or Osteopathy Without License A Class C felony carries one year and one day to 10 years in prison and a fine of up to $15,000.6Alabama Legislature. Code of Alabama 13A-5-11
The setting does not change the analysis. A medical spa, a private home, and a “Botox party” at a salon are legally identical for this purpose. If the person holding the syringe lacks the license, the injection is a felony.
How to Check a Provider Before Your Appointment
The Alabama Board of Medical Examiners and Medical Licensure Commission maintains a public license lookup. You can search by name, license number, or license type, and the result shows current status, including whether a license is active, on probation, suspended, or revoked. The database covers MDs, DOs, PAs, and CRNPs.7Alabama Board of Medical Examiners & Medical Licensure Commission. License Lookup
Before the appointment, ask the provider directly: what is your license type, who is the collaborating or supervising physician if applicable, and is the approved ABME protocol on file? A legitimate provider will answer without hesitation.
